Primary Care Nurse Practitioner (FNP) or Physician Assistant (PA-C) — Direct Primary Care

Location: Tyler, Texas

Job Type: Full-time, in-office

Job Description

An established Direct Primary Care practice in Tyler, Texas is seeking a patient-centered Family Nurse Practitioner or Physician Assistant to provide comprehensive family primary care. This is an opportunity to practice in a relationship-driven setting where providers have the time to know their patients, address concerns thoroughly, and deliver thoughtful care without a high-volume, insurance-based workflow.

The role includes preventive care, chronic-disease management, acute visits, lab review, prescription follow-up, and coordination of care for patients from age 1 through adulthood. Appointments are typically scheduled for one hour, allowing providers to see approximately seven to eight scheduled patients per day while also addressing occasional urgent needs and patient follow-up.

The practice provides each provider with a dedicated Medical Assistant or LVN, uses the Elation EHR with AI-assisted charting, and draws labs in-house for send-out testing. The ideal candidate is a clinically confident primary care provider who values continuity, excellent communication, and an unhurried patient experience.

Key Responsibilities

Deliver comprehensive primary care, preventive services, and chronic-disease management for a family patient population.
Evaluate and treat routine acute concerns and address occasional same-day patient needs within the scope of primary care.
Conduct thorough, patient-centered visits; interpret labs and diagnostic information; and develop appropriate care and follow-up plans.
Manage patient messages, medication refills, lab reviews, care coordination, and referrals in a timely manner.
Perform routine in-office primary-care procedures consistent with training, credentialing, and clinic protocols.
Collaborate closely with the physician, advanced-practice-provider, and dedicated clinical-support team to ensure a consistent patient experience.

Qualifications

Active Texas license as a Nurse Practitioner or Physician Assistant.
Current DEA registration and BLS/ACLS certification.
At least 2–3 years of relevant experience in family medicine, primary care, internal medicine, or urgent care preferred.
Comfort caring for patients across the lifespan, including pediatric patients.
Strong clinical judgment, communication skills, and ability to work independently within a collaborative team.
Experience with common outpatient procedures, chronic-condition management, and EHR documentation is preferred.

Compensation and Benefits

The practice offers a competitive compensation and benefits package, including:

Base salary of $115,000–$130,000, commensurate with experience.
Paid Malpractice
Paid DEA, licensing and dues
Dedicated MA or LVN support for each provider.
Elation EHR with AI-assisted charting to help reduce documentation burden.
A manageable, relationship-focused patient schedule and the professional satisfaction of delivering accessible primary care.

Schedule

Monday–Friday, 8:00 AM–5:00 PM with lunch break.
Saturday and Sunday off.
