Job Summary
The Health Navigator will engage directly with individuals who are at risk for HIV, hepatitis C (HCV) and opioid overdose; providing services that help to reduce the risks of acquisition and transmission of infectious disease, by encouraging practices that reduce harm.
This person will provide services through the Syringe Services Program. The primary duties are to dispense substance use supplies, as well as overdose prevention materials and education. This person will also be engaged in life skills counseling and testing for HIV and HCV, linkage to medical and recovery services, and to engage individuals at all levels and stages of change. This person will also conduct outreach activities in the field and at community events.
Specific duties may include:
1. Implement effective community health services and outreach for people who engage in highrisk activities; which may include substance use through injection and other methods of use, and who engage in sexual activities
2. Develop trusting relationships with PWUD (people who use drugs).
3. Distribute supplies that reduce the risk of health harms to targeted areas in Mendocino County on a regular schedule as assigned;
4. Pick up and transport used supplies back to office;
5. Provide training for and distribution of naloxone for overdose prevention;
6. Collect overdose reversal information;
7. Provide information, referral and direct linkage to medical, behavioral and human services resources;
8. Provide motivation and recovery/treatment resources applicable and appropriate for levels of readiness and stages of change;
9. Attend trainings and applicable meetings as directed by supervisor.
10.Prepare, maintain, and submit participant reports that are timely and accurate;
11.Participate in staff and agency meetings as directed by supervisor;
12.Provide counseling and testing both in-house and in strategic outreach sites identified as having individuals at risk;
13.Occasional client transportation;
14.Maintain mileage records for appropriate reimbursement of fuel costs

Pay: From $20.00 per hour
