Full Job Description

JOB SUMMARY:

The Madrone Care Network (MCN) Care Coordinator/Certified Medical Assistant plays a critical, patient-facing role as one of the first lines of support in a compassionate, high-touch care delivery model. Acting as a key point of contact between patients, caregivers, healthcare providers, and internal care teams, the Care Coordinator is responsible for delivering exceptional service through proactive, personalized outreach and timely administrative support.

This role is central to ensuring a seamless patient experience—building trust, alleviating confusion, and coordinating care across multiple stakeholders. The Care Coordinator facilitates clear and empathetic communication, connecting with patients and caregivers via phone, secure messaging, and other communication platforms to help them navigate care pathways, understand next steps, and feel fully supported throughout their health journey.

In general, this position works Monday - Friday from 8:30 AM - 5:00 PM.

ESSENTIAL JOB FUNCTIONS:

Patient Communication & Support - Provides proactive patient support by scheduling in-home and telehealth visits, making reminder calls, arranging transportation, and assisting with the completion and signing of patient forms as requested by staff.

2. Clinical Documentation & Records Management - Documents all patient interactions in the electronic health record (EHR), monitors the EHR inbox, scans files and documents into patient charts, requests and obtains medical records, and maintains the Master Patient List—including auditing visit frequency requirements.

3. Care Team Coordination - Supports care team members by creating and managing appointment calendars, assisting with teleconferencing and visit technology, ordering Durable Medical Equipment (DME), and facilitating communication across the team.

4. Meeting & Office Support - Takes notes during Interdisciplinary Team (IDT) meetings, schedules external meetings as requested, and contributes to the efficient day-to-day functioning of the office.

5. Phone & Frontline Coverage - Provides phone coverage when team members are on calls or breaks and supports overall customer service and communication continuity for the team and patients.

7. Support day-to-day operations and perform other duties as assigned, aligned with patient, caregiver, and community needs—and in service of the organization’s commitment to walk with patients as they live the remainder of their lives.

PHYSICAL REQUIREMENTS:

Work is primarily performed in an office environment that is air-conditioned.

Ability to sit for extended periods, as well as frequent walking, standing, bending, and reaching.

Ability to hear and communicate effectively verbally and in writing with patients, caregivers, team members, and community partners, with or without reasonable accommodation.

Ability to read, review, and analyze written documents and electronic records.

Ability to lift, carry, or move items weighing up to 10lbs.

QUALIFICATIONS:

Current California Driver’s License, auto insurance and vehicle registration
1 Year related professional work experience
Bi-Lingual in Spanish
Advanced computer and technology skills. Experience in electronic medical records is preferred.
Capacity to remain organized, prioritize and multi-task and work independently with minimum supervision.
Ability to demonstrate respect and professionalism toward all patients, caregivers, and colleagues, regardless of their circumstances, ethnicity, race, sexual orientation, gender identity, religious beliefs, or political affiliation.
