Full Job Description

Bilingual Case Manager - Adults with Disabilities | Ukiah, CA $28–$30/hr | Full-time | Spanish required

Lead a caseload supporting adults with developmental disabilities and their families across the Ukiah/Fort Bragg area. You'll coordinate services across two programs - one focused on family-based care coordination, the other on helping adults live more independently in the community.

Note: This role includes local travel in the Ukiah/Fort Bragg area.

In this role, you’ll help coordinate care, support teams, and ensure clients are receiving high-quality, person-centered services - while building strong relationships with families and community partners.

What You’ll Do

Manage a caseload across both CFS and ILS programs
Support and guide caregivers and team members
Partner with families to help clients reach their goals
Coordinate services, schedules, and documentation
Ensure compliance with program requirements and quality standards
Travel throughout your assigned region to meet with clients and teams

What You’ll Bring

Bachelor’s degree required
Bilingual (Spanish)
Experience in social services, case management, or working with individuals with developmental disabilities
Strong communication, organization, and leadership skills
Ability to manage multiple priorities and work independently
Valid driver’s license, reliable vehicle, and willingness to travel locally
