Job Title: Treatment Coordinator/QIDP

Program: OPWDD Residential Services
Reports To: Residential Area Coordinator
FLSA Status: Exempt

Position Summary

The Treatment Coordinator/QIDP is responsible for coordinating, monitoring, and ensuring the implementation of individualized treatment plans for individuals residing in OPWDD-certified residential homes. This role ensures that services are person-centered, compliant with OPWDD regulations, and aligned with each individual’s goals, needs, and safeguards. The Treatment Coordinator works collaboratively with interdisciplinary team (IDT) members, residential staff, families, Care Coordination Organizations (CCOs), and external providers.

This position carries responsibilities equivalent to those of a Residential Supervisor, including oversight of treatment plan implementation, staff guidance, and ensuring program compliance with OPWDD regulations.

Essential Duties and Responsibilities

Treatment Planning & Compliance

Coordinate the development, implementation, and monitoring of individualized treatment plans in accordance with OPWDD regulations and agency policies
Ensure treatment plans are person-centered, outcomes-focused, and reflective of the individual’s strengths, preferences, and needs
Monitor plan effectiveness and initiate updates or revisions as required
Ensure timely completion of required documentation, including Life Plans, addendums, assessments, and progress reviews

Interdisciplinary Team Coordination

Schedule, prepare for, and facilitate IDT meetings, including Life Plan meetings and periodic reviews
Collaborate with clinical staff, residential management, Direct Support Professionals (DSPs), nurses, behavior specialists, and external providers
Communicate treatment plan expectations clearly to residential staff and provide guidance on implementation

Documentation & Record Review

Review treatment plans, progress notes, and related documentation for accuracy, completeness, and regulatory compliance
Track due dates and ensure all required documentation is completed within OPWDD timelines
Maintain organized, audit-ready records in both electronic and paper formats

Staff Support & Training

Provide training and technical assistance to residential staff on treatment plan goals, strategies, and documentation requirements
Support supervisors in monitoring staff implementation of treatment plans
Identify areas where additional training or support is needed and communicate concerns to management

Quality Assurance & Oversight

Conduct periodic reviews of treatment plan implementation in residential settings
Participate in internal audits, quality improvement activities, and OPWDD surveys
Address deficiencies and assist in developing corrective action plans as needed

Coordination with External Partners

Liaise with Care Coordination Organizations (CCOs), clinicians, day programs, and other service providers
Support communication with families and advocates regarding treatment planning and progress

Supervisory and Program Oversight

Provide leadership and support to residential staff in implementing treatment plans and maintaining program standards. Collaborate with Supervisors and Area Coordinators to ensure continuity of care and operational compliance across multiple residences.

Qualifications

Education

Bachelor’s degree in Human Services, Social Work, Psychology, Special Education, or a related field required

Experience

Minimum of two (2) years of experience working with individuals with intellectual and/or developmental disabilities
Experience in supervisory responsibilities within OPWDD residential programs required
Knowledge of person-centered planning and Life Plan requirements

Knowledge, Skills & Abilities

Strong knowledge of OPWDD regulations, treatment planning requirements, and documentation standards
Excellent organizational, time management, and attention-to-detail skills
Ability to work collaboratively with multidisciplinary teams
Strong written and verbal communication skills
Proficiency with electronic health records and Microsoft Office
Ability to manage multiple homes, priorities, and deadlines

Work Requirements

Ability to travel between residential homes in Queens and attend meetings
Evening or weekend hours as needed for meetings or program needs
