Full Job Description

FULL TIME- 37.5 hours per week

DUTIES: Fayette County Drug and Alcohol Commission is currently accepting applications for a full-time Case Management Specialist to join our Case Management Unit. The Case Management Unit typically is the first point of interaction with individuals seeking assistance with substance use disorders. The Case Management Specialist works with individuals who are either self-referred or referred from agencies such as, County or Federal Probation, State Parole, CYS agencies, Mental Health Providers, Physicians, Hospitals, other Substance Use Treatment Providers, Courts, Juvenile Placements and School Districts. The role of the Case Management Specialist is to complete screenings, level of care assessments, make referrals and coordinate placement for treatment services. Assessments may occur in settings such as, local hospitals, medical provider offices, law enforcement offices, school settings, the local county jail, and other social service agencies. The Case Management Specialist is responsible for all aspects of drug and alcohol case management record keeping as required by the agency, Department of Drug and Alcohol Programs, as well as third party payers. This position requires working at least one evening a week until 7PM.

REQUIREMENTS: Bachelors or Master’s degree with a major in a behavioral health or related field and at least 1 year of clinical experience. Strong interpersonal skills and strengths in the areas of organization and documentation are required, including familiarity of Excel spreadsheets. Applicants must be able to work and think independently with minimal supervision with a wide variety of clientele. Must have a valid driver’s license, insurance, and reliable transportation. Required to obtain the Allied Addiction Practitioner Certification through the Pennsylvania Certification Board, when the individual is eligible based on training and experience.
