About Living Hope Clinic

Living Hope Clinic is a dynamic, integrated outpatient facility in Phoenix, Arizona, committed to supporting individuals and families in healing physically, mentally, and spiritually. Our mission-driven team delivers holistic care through both behavioral health and primary medical services, addressing root causes of addiction, mental illness, and systemic family issues. With a strong focus on personalized treatment, prevention, and accessibility—including telehealth and mobile services—we are dedicated to empowering our clients toward lasting wellness and recovery.

Position Summary

Living Hope Clinic is seeking a compassionate and qualified Family Nurse Practitioner (FNP) licensed in Arizona to join our growing team on a part-time basis, with flexible scheduling and opportunities for expanded hours as caseloads increase. This is a great opportunity for a Nurse Practitioner looking to supplement their current role or gradually transition into a more permanent clinical setting. You’ll play a critical role in providing patient-centered primary care services and collaborating with our behavioral health professionals to support holistic client wellness.

Key Responsibilities

Provide outpatient primary care, wellness visits, and follow-ups for a diverse patient population.
Conduct comprehensive physical assessments, diagnose health conditions, and formulate treatment plans.
Manage chronic illnesses, preventive care, health screenings, and wellness education.
Order and interpret diagnostic tests (e.g., labs, urinalysis, glucose monitoring, etc.).
Prescribe and manage appropriate medications in accordance with Arizona law.
Provide minor clinical procedures (e.g., wound care, injections, blood pressure monitoring, etc.).
Collaborate with the behavioral health team for integrated care coordination.
Maintain accurate documentation in Electronic Health Records (EHR) and follow HIPAA regulations.
Participate in mobile or telehealth services when needed.

Qualifications

Current Arizona Nurse Practitioner license and national certification as a Family Nurse Practitioner (FNP-BC or FNP-C).
Dual certification as a Family Nurse Practitioner (FNP) and Psychiatric-Mental Health Nurse Practitioner (PMHNP) is highly desirable, but not required.
Minimum of 1–2 years of experience in primary care, urgent care, or outpatient settings preferred.
Familiarity or interest in working with clients experiencing behavioral health or substance use issues is a plus.
Excellent clinical, communication, and documentation skills.
Ability to work independently and collaboratively in an interdisciplinary team environment.
CPR/BLS certification (required).

Compensation & Schedule

Hourly Rate: Competitive and based on experience $ 60-$65 an hour
Schedule: Flexible—build your own hours around your existing commitments. Ideal for NPs working other routes or positions.
Initial schedule of 5 hours per week, with increased hours as client volume grows.
