Job Title: Weekend Home Health Intake Coordinator | Admissions & Referrals

Location: Golden Valley, Minnesota

Compensation: $24-$28 per hour

Schedule: Part-Time | Friday-Sunday or Friday-Monday | Every Weekend Preferred | Every Other Weekend Considered

Training: Monday-Friday | 8:00 a.m. - 5:00 p.m. for the first several weeks

At Home Health Care, Inc., we're hiring a Weekend Home Health Intake Coordinator for a Friday-Sunday or Friday-Monday schedule.

You'll still build valuable healthcare experience.

You'll still work with hospitals, clinics, physicians, nurses, therapists, patients, and families.

You'll just build your workweek differently.

Important: Initial training takes place Monday-Friday from 8:00 a.m. - 5:00 p.m. for the first several weeks. Candidates must be able to accommodate weekday training before transitioning into the weekend schedule.

More Than Scheduling. You're Moving Admissions Forward.

When a home health referral arrives, someone needs to determine what happens next.

Is the referral complete?

Do we have the appropriate orders?

Is the patient eligible?

Is documentation missing?

Does the referral source need to be contacted?

Does the clinical team have everything necessary to move toward admission?

That's the work you'll help coordinate.

You'll take incoming referrals and help move them from "received" to "ready for care."

What You'll Do

Review incoming home health referrals.
Verify insurance, payer, and reimbursement eligibility.
Review physician orders and supporting clinical documentation.
Identify missing information and proactively obtain it.
Communicate with hospitals, clinics, physician offices, and other referral sources.
Communicate with patients and families regarding the intake process.
Enter accurate information into the EMR.
Track referrals through the admission process.
Coordinate with nursing, therapy, intake, and operational teams.
Prioritize multiple referrals while maintaining accuracy and responsiveness.

If you've spent time as a Health Unit Coordinator, you already know what it's like to have:

Health Unit Coordinator (HUC).
Unit Coordinator or Health Unit Clerk.
Home Health Intake Coordinator.

Medical Office Coordinator.
Patient Administration Specialist.
Healthcare Administrative Professional.

Previous home health intake or admissions experience is strongly preferred, but healthcare professionals with directly transferable experience are encouraged to apply.

Strong understanding of medical terminology and healthcare workflows.
Excellent computer and EMR skills.
Strong attention to detail.
Ability to manage several priorities simultaneously.
Professional communication with clinical teams and referral sources.
Persistence when information or documentation is missing.
Strong problem-solving and follow-through.

Experience with home health admissions, referrals, insurance verification, payer guidelines, Medicare requirements, or healthcare intake is highly preferred.
