Full-Time Director of Patient Services (DPS) - 90% Office & 10% Field Based

Licensed Home Care Services Agency (LHCSA) – $155,000 to $160,000 + Benefits

Celebrating 12 years of exceptional care, This LHCSA Home Care Agency operates under a clear guiding philosophy: “We aren’t looking to be the biggest – we are looking to be the best.” Serving a robust census of over 350 Medicaid, private pay, and CHHA-contracted patients, this organization is seeking a dedicated, compliance-driven nurse leader to partner directly with the Administrator to maintain clinical excellence and support their ongoing regional expansion.

What You’ll Do: Clinical Leadership, Compliance & Audit Readiness

As the clinical cornerstone of the agency, you will oversee all clinical operations, ensure strict adherence to New York State Department of Health (DOH) regulations, and maintain absolute readiness for state audits. This is a highly collaborative, hands-on role requiring a blend of administrative oversight, private-pay customer service, and clinical team mentorship.

Lead and manage all clinical aspects of the LHCSA, including:

Audit & Compliance Management: Maintain continuous survey readiness, review clinical documentation, monitor continuous quality improvement (CQI) indicators, and proactively implement systems to prevent DOH citations.
Team Supervision & Oversight: Directly manage 3 Coordinators and 2 Per Diem Field Nurses, while providing high-level clinical guidance for a workforce of over 700 Home Health Aides (HHAs).
Clinical Quality & Assessments: Oversee the intake, assessment, and reassessment processes. Utilize a specialized digital training platform to track and monitor HHA in-service compliance (no manual curriculum instruction required).
Patient & Operational Advocacy: Balance the regulatory needs of Medicaid cases with the high-touch, service-oriented expectations of private pay clients. Step in to conduct virtual visits or field supervision as needed to resolve complex care issues.
Systems & Documentation: Utilize HHA Exchange EMR to drive clinical documentation accuracy, oversight, and operational reporting.

Schedule, Location & Hybrid Flexibility

Work Week: Monday – Friday, 9:00 AM – 5:00 PM (with the natural flexibility to work earlier or later based on operational demands).
On-Call Accountability: 24-hour executive accountability for clinical matters. A company phone is provided; on-call triage responsibilities are shared collaboratively with other agency leaders via phone.
Current Address: 1820 Gravesend Neck Road, Brooklyn, New York 11229 (Street parking available).
Hybrid / Remote Policy: 100% on-site during the initial training and probationary period. Once fully settled and established, the agency offers a flexible 2-day work-from-home (WFH) schedule for administrative tasks.
Field Travel: Light travel across the 5 boroughs (excluding Staten Island), with minor oversight for recent expansions into Nassau County and Westchester.

Who You Are: Credentials & Qualifications

Licensure & Education: Active New York State Registered Nurse (RN) license. A Bachelor of Science in Nursing (BSN) is highly preferred.
Experience Requirements:
Minimum of 1 year of current experience as a DPS within a LHCSA or CHHA setting, OR
A high-performing Supervisor/Clinical Manager within a LHCSA who has directly assisted a DPS, possesses extensive audit experience, and is ready to take a confident step up into executive leadership.
Stability: A stable, consistent career history is required (no job-hopping or "jumpy" resumes). There is no formal training framework for this executive role; you must hit the ground running.
Specialized Knowledge: A proven track record of successfully navigating DOH audits is a MUST. Familiarity with Nursing Home Transition and Diversion (NHTD) waiver programs and private pay service management is highly preferred.
Bilingual Skills: Bilingual language skills are highly valued and will position candidates at the top of the salary framework.

Compensation & Benefits

Salary Range: $155,000 to $160,000
