Overview

Our mission is to make the community pharmacy the center of care - where primary care may not be as readily available for uninsured or individuals where primary care is not easily accessible.

Join our dynamic healthcare team as a Telehealth Primary Care Provider, where you will deliver comprehensive, patient-centered medical care through innovative telehealth platforms. This role offers the opportunity to improve health outcomes by providing accessible, high-quality primary care services remotely. You will collaborate with a multidisciplinary team of healthcare professionals to ensure seamless patient management, diagnosis, and treatment planning in a fast-paced, technology-driven environment. Your expertise will help bridge gaps in healthcare access and foster healthier communities.

Duties

MD is required, board certified. No exceptions.
Conduct thorough virtual patient assessments, including vital signs, medical history review, and physical examinations via telehealth systems.
Prescribe medications and treatment plans in accordance with HIPAA regulations and DEA guidelines, ensuring safe medication management.
Utilize EMR/EHR systems such as Epic and eClinicalWorks to document patient encounters accurately and efficiently.
Collaborate with specialists and other healthcare providers to coordinate care for complex cases, including those requiring critical care or hospital-level interventions.
Manage urgent and non-urgent cases across diverse populations, including pediatrics, geriatrics, behavioral health, and patients with chronic conditions.
Perform diagnostic evaluations using telehealth-compatible tools and order appropriate laboratory tests or imaging studies when necessary.
Provide health coaching and education to empower patients in managing their health and wellness effectively.

Requirements

Valid medical license to practice as a primary care provider in the designated state or region.
Multiple state licenses is required, in particular in states such as CA, TX, PA, WI, OH, FL, GA, TN, LA, PA, SC and other East Coast states.
Proven experience working with EMR/EHR systems
Demonstrated telehealth experience with proficiency in virtual patient engagement tools and platforms.
Strong clinical skills across internal medicine, pediatrics, geriatrics, and behavioral health disciplines.
Knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques.
Ability to perform physical examinations remotely and interpret diagnostic data accurately.
Familiarity with CPT coding, ICD coding (ICD-10), drug enforcement regulations (DEA), and documentation standards for telehealth services.
Excellent client communication skills with a compassionate approach to diverse patient populations.
Critical care experience or hospital medicine background is highly desirable; experience working with individuals with disabilities or developmental disabilities is a plus.
