Rate of Pay: $27.47 /hour plus a $2,000 sign on bonus
Base Schedule: 35-hour work week. Monday – Friday 8:30-4:30. With occasional nights and weekends including rotation of on call.
JOB DESCRIPTION: The Intensive Case Planner provides comprehensive, strength-based case management services to families involved with the child welfare system. This role is focused on ensuring child safety, family stabilization, and long-term well-being through collaborative planning, advocacy, and therapeutic support. The Intensive Case Planner works closely with families, the Department of Family and Community Services (DFCS), legal stakeholders, schools, mental health providers, and other community-based organizations.
SPECIFIC RESPONSIBILITIES INCLUDE:
- Maintain a caseload of families with elevated service needs and/or high-risk indicators.
- Conduct regular home and community-based visits (minimum twice per month or as mandated).
- Engage children and families using a trauma-informed and culturally responsive approach.
- Complete comprehensive family assessments and individualized service plans in accordance with DFCS.
- Coordinate services including mental health, substance use, domestic violence, education, housing, and employment supports.
- Facilitate family team conferences and case planning meetings with internal and external stakeholders.
- Maintain timely and accurate documentation in electronic case records (e.g., Connections).
- Submit all required reports, progress notes, and court reports in accordance with DFCS and agency guidelines.
- Ensure all contacts, consents, and assessments meet regulatory and agency standards.
- Collaborate with DFCS, legal representatives, and service providers to ensure coordinated care.
- Coordinate with clients and treatment providers to secure placement for children requiring secured detention.
- Attend and provide testimony at Family Court proceedings when required.
- Advocate for services that strengthen family functioning and promote child well-being.
- Respond promptly to emergencies, including safety concerns, hospitalizations, or child removals.
- Work collaboratively with supervisors to assess risk and implement safety interventions.
- Participate in individual and group supervision.
- Attend case conferences, training sessions, and team meetings.
- Maintain certification or proficiency in evidence-based models and/or mandated programs (Mandated Reporting).
- Maintains self in a professional manner at all times and adheres to the policies and procedures of the Agency.
- Performs various related tasks as assigned or required.
- Punctual and regular attendance is an essential responsibility of each employee. Employees are expected to report to work as scheduled, on time and prepared to start working.
- Employees are expected to be available to work occasional evening and weekend events as assigned, based on the needs of the program.
- Employee must be available to participate in a rotating on-call schedules as required by the program. This includes responding to urgent matters outside of regular business hours, including evenings, weekends, and holidays.
REQUIRED QUALIFICATIONS:
- A Bachelor’s Degree in Social Work, Nursing, Psychology or a closely related Human Service field
- AND
- Two (2) years relevant experience working with children/adults with mental health/ behavioral health concerns
AND
- A valid New York State Driver’s License and reliable transportation required to conduct home/community based visits
AND
- Knowledge of DFCS Preventive Services model and evidence-based interventions.
- Familiarity with state and local child welfare regulations and systems.
- Proven strong organizational, assessment and communication skills.
- Experience in securing cooperation of, and working effectively with individuals and agencies in the community.
