DEPARTMENT: Residential Services DATE: 10/24JOB TITLE: Residence Counselor BAND: 2 STATUS: Non-Exempt

Position Summary

The Residence Counselor provides direct care, supervision, skill development, and emotional support to individuals residing in agency-operated residential programs. The position ensures health, safety, and dignity while supporting individuals to achieve personal goals consistent with person-centered service plans, applicable regulations, and agency values.

QUALIFICATIONS

Education

A minimum of a high school diploma or equivalent.

Experience

A minimum of one-year experience working with the mental health and substance abuse population preferred.

Skills

Demonstrated ability to establish and maintain in a professional manner relationship with clientele and personnel within and outside the Agency.
Proven skills in household maintenance, including cooking, cleaning, and other daily living tasks.
Strong problem-solving abilities and excellent communication skills.
Must demonstrate those general skills and personality traits deemed necessary to successfully meet the demands of the position.
Strong reading and writing skills.
A valid New York State Driver’s License with 5 points or less.

RESPONSIBLE TO

Residence Manager

MAJOR FUNCTIONS

Administrative

Required to follow all Agency policies and procedures, OMH regulations and Justice Center standards.

· Required to understand and abide by those laws, regulations, policies, and procedures that are applicable to the program/service which you are providing.

Immediately report suspected or actual incidents, abuse, neglect, or significant events in accordance with OMH and Justice Center requirements.
Required to maintain accurate statistical records and files on program participants and activities, including but not limited to individual client records, billing records, medication records, daily log, expense sheets, purchase orders, petty cash, client funds, mileage records, and menus.

Client/Program Related

Provide guidance and support to individuals living in a residential facility.
Monitor and evaluate the progress of individuals.
Develop and implement strategies to support the needs and goals of the residents.
Ensure compliance with applicable laws and regulations.
Maintain records and document progress notes.
Provide crisis intervention and de-escalation support.
Assist with daily living activities.
Facilitate group activities and programs.
Assist with the development of life skills.
Grocery shopping for residence.
Cooking with/for clients.
Cleaning, assist clients with house chores.
Develop and implement service plans and service plan reviews.
Supervise self-medication administration and provide assistance as needed. (schedule appointments and order medication) -Community Residences
Support resident’s efforts to self-administer medications and monitor compliance with med regime during scheduled home visits. -Pathways
Provide transportation to/from appointments and other activities as needed.
Collaborate with program/treatment teams and other professionals to ensure continuity of care.
Performs other duties as assigned.

Catholic Charities Agency and Community Related

Attend staff meetings on a regular basis.
Participate in service training sessions, workshops, and/or conference within Catholic Charities and the larger community, as necessary.
Perform other duties within the organization, as directed.
