Full Job Description

Tropical Breeze Health Care is seeking a compassionate, motivated, and patient-centered Bilingual Advanced Practice Registered Nurse (APRN) to join our growing mobile healthcare practice serving Southwest Florida.

This position offers the opportunity to provide comprehensive, high-quality medical care directly in patients’ homes while working alongside an experienced multidisciplinary team dedicated to improving patient outcomes and reducing unnecessary hospitalizations.

The ideal candidate is passionate about delivering personalized, evidence-based care and enjoys building meaningful relationships with patients in their home environment.

Position Summary

The APRN will provide comprehensive mobile healthcare services, including Primary Care, Wound Care, Telehealth, Wellness Services, and ongoing chronic disease management for adult and geriatric patients throughout our service area.

This position will initially begin as part-time (approximately 20 hours per week) with the opportunity to transition into a full-time position as the practice continues to grow.

Responsibilities

* Perform comprehensive patient assessments and physical examinations.

* Provide Mobile Primary Care services in patients’ homes.

* Diagnose, treat, and manage acute and chronic medical conditions.

* Develop individualized treatment plans.

* Perform follow-up visits and routine re-evaluations.

* Provide comprehensive wound care management in collaboration with the Medical Director and according to Tropical Breeze Health Care clinical protocols.

* Perform wound assessments, measurements, documentation, dressing changes, and monitor healing progress.

* Manage patients receiving Negative Pressure Wound Therapy (NPWT) when clinically appropriate.

* Administer provider-prescribed injections and wellness therapies.

* Conduct Telehealth visits for appropriate follow-up care.

* Review laboratory and diagnostic results.

* Perform medication reconciliation and medication management.

* Educate patients and caregivers regarding disease management and treatment plans.

* Coordinate care with physicians, specialists, hospitals, home health agencies, and other healthcare providers.

* Document all patient encounters accurately using the electronic medical record (EMR).

* Participate in quality improvement initiatives and maintain compliance with all federal and state regulations.

Qualifications

* Active Florida APRN License.

* Board Certified Family Nurse Practitioner (FNP).

* BLS Certification.

* DEA License (or ability to obtain).

* Bilingual (English & Spanish) required.

* Valid Florida Driver’s License and reliable transportation.

* Comfortable traveling to patient homes throughout the service area.

* Excellent communication and organizational skills.

* Strong clinical judgment and ability to work independently.

* Experience in Primary Care, Geriatrics, Home Visits, Wound Care, or Internal Medicine is preferred but not required.
