Full Job Description

Position Summary
The Housing Stability Case Manager works directly with individuals and families experiencing or at risk of homelessness to help them achieve long-term housing stability. Many participants may be living with mental health conditions, substance use disorders, chronic health conditions, or other barriers that impact housing stability. This position provides individualized case management, develops service plans, connects participants with community resources, and works collaboratively with landlords, healthcare providers, behavioral health providers, employers, and community partners to help clients reach their goals.

The ideal candidate is passionate about helping others, thrives in a fast-paced environment, and believes in a strengths-based, client-centered, trauma-informed approach.

Essential Responsibilities

Conduct comprehensive assessments to identify client strengths, needs, barriers, and housing goals.
Develop individualized housing stabilization and service plans.
Assist participants with obtaining and maintaining safe, stable housing.
Connect clients to community resources, including physical healthcare, mental health treatment, substance use treatment and recovery supports, employment services, education, transportation, and public benefits.
Provide crisis intervention, advocacy, supportive counseling, and ongoing encouragement to promote housing stability.
Support participants in navigating complex systems while respecting client choice and self-determination.
Coordinate services with behavioral health providers, hospitals, Veterans Affairs providers, and other community partners as appropriate.
Monitor participant progress and adjust service plans as needed.
Maintain accurate, timely, and complete electronic case records.
Participate in case staffing meetings and interdisciplinary team discussions.
Ensure compliance with program policies, funding requirements, and agency standards.

Qualifications

Preferred

Bachelor's degree in Social Work, Psychology, Human Services, Sociology, Counseling, or a related field. Relevant combination of education and directly related experience may be considered.
Minimum of one (1) year of experience providing case management, care coordination, or supportive services to individuals or families.
Experience working with vulnerable populations, including individuals experiencing homelessness, veterans, or individuals living with mental health conditions, substance use disorders, or other complex barriers to housing stability.
Knowledge of trauma-informed care, person-centered planning, crisis intervention, and strengths-based service delivery.
Excellent interpersonal, written, and verbal communication skills with the ability to build rapport with diverse populations.
Strong organizational skills with the ability to manage multiple participants, prioritize competing demands, and maintain timely documentation.
Proficiency with Microsoft Office (Word, Excel, Outlook) and the ability to learn electronic case management systems, including HMIS.
Valid Florida driver's license, reliable transportation, and proof of automobile insurance.
