Mission Support

The Housing Support Program Coordinator plays a critical role in supporting Attentive Care Inc.’s Housing Support Program by ensuring efficient program operations, timely participant services, accurate documentation, and successful coordination of housing resources that promote housing stability and independence.

Position Summary

The Housing Support Program Office Assistant provides administrative and operational support to the Housing Support Program. This position is responsible for coordinating referrals, managing client records, tracking authorizations, scheduling appointments, maintaining documentation, and supporting Housing Specialists to ensure participants receive timely and effective housing support services.

Duties and Responsibilities:

Answer and route incoming calls, emails, and inquiries from participants, referral sources, and community partners.
Maintain client files and ensure all required documentation is complete and up to date.
Prepare correspondence, reports, forms, and program-related documents.
Schedule appointments, home visits, and meetings for Housing Specialists and program staff.
Maintain confidential records in accordance with HIPAA and agency policies.
Receive and process referrals from managed care organizations, hospitals, community agencies, and other referral sources.
Assist with intake documentation and collection of required participant information.
Track referral status from receipt through program enrollment.
Coordinate communication between participants, Housing Specialists, and referral sources.
Monitor authorizations and service approvals from managed care organizations.
Track authorization start and end dates and notify staff of renewals.
Ensure required assessments, service plans, progress notes, and supporting documentation are submitted timely.
Assist with data entry and quality assurance reviews.
Maintain housing resource lists, landlord contacts, and community partner information.
Assist with tracking housing applications, inspections, lease agreements, and housing placements.
Monitor participant service logs and contact sheets.
Support HMIS data entry and reporting requirements.
Generate reports related to participant outcomes, referrals, and housing placements.
Ensure compliance with agency, state, Medicaid, and HUD requirements.
Assist with audits, monitoring reviews, and record requests.
Track required staff documentation and training records.
Maintain accurate and organized program records.

Qualifications:

Associate degree in Business Administration, Human Services, Social Work, or related field preferred.
Minimum of 3 years of administrative experience in healthcare, social services, housing services, or a related field.
Experience with electronic records systems, HMIS, or Medicaid documentation preferred.
Proficiency in Microsoft Office Suite (Word, Excel, Outlook).
Strong organizational and time-management skills.
Excellent customer service and communication skills.
Ability to maintain confidentiality and handle sensitive information.
Strong attention to detail and accuracy.
Ability to prioritize multiple tasks and meet deadlines.
Ability to work independently and collaboratively with staff and community partners.

Preferred Experience

Knowledge of supportive housing, homelessness prevention, and housing resources.
Experience working with Medicaid-funded programs and managed care organizations.
Familiarity with housing applications, benefits coordination, and community-based services.

Work Schedule

Full-Time
Monday – Friday
8:30 AM – 5:00 PM with an hour un paid lunch break

Pay: $20.00 - $25.00 per hour
