Cass County Senior Living & Rehabilitation is seeking an experienced Registered Nurse (RN) to join our clinical leadership team as our MDS Nurse / Infection Preventionist.
This position is ideal for an RN who enjoys the clinical side of nursing but is ready for a role focused on quality, compliance, resident outcomes, and leadership.

This unique position combines MDS coordination and Infection Prevention, giving you the opportunity to make an impact across multiple areas of resident care.
You'll work closely with nursing leadership and the interdisciplinary team to support accurate MDS assessments, care planning, reimbursement, quality measures, infection prevention, regulatory compliance, and positive resident outcomes.
Coordinate and complete MDS assessments
Ensure accurate and timely documentation
Support individualized resident care planning
Monitor coding, reimbursement, and quality measures
Collaborate with the interdisciplinary care team
Coordinate the facility's Infection Prevention and Control Program
Conduct infection surveillance, tracking, and required reporting
Monitor infection control practices throughout the facility
Educate and support staff on infection prevention practices
Assist with regulatory compliance and survey readiness
Complete periodic off-shift observations as needed
Current Illinois Registered Nurse (RN) license required
Previous MDS experience required
Infection Prevention experience preferred, but not required
Willingness to complete Infection Preventionist training if needed
Long-term care experience preferred
Strong organizational and communication skills
Excellent attention to detail
Ability to work independently and as part of a leadership team
