Full Job Description

Overview
We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join Blake team. The ideal candidate will have a strong commitment to providing high-quality patient care and will possess the skills necessary to support residents with diverse healthcare needs. As an LVN, you will play a vital role in the assessment, planning, and implementation of residents care plans, ensuring that all residents receive the attention and support they require.

Responsibilities

Provide direct patient care under the supervision of registered nurses and physicians.
Assist in the development and implementation of individualized care plans.
Monitor residents’ health status and report any changes to the healthcare team.
Administer medications and treatments as prescribed by physicians.
Perform diagnostic evaluations and assist with specimen collection processing.
Support residents with acute pain management strategies.
Engage in case management activities to coordinate patient care services.
Maintain compliance with HIPAA regulations to protect patient confidentiality.
Document all residents interactions accurately in accordance with facility policies.
Participate in hospice care initiatives for terminally ill patients as needed.
Provide assistance in assisted living environments, ensuring a safe and supportive atmosphere.

Skills

Experience working with individuals with developmental disabilities is highly desirable.
Proficient in acute pain management techniques.
Ability to lift residents safely and effectively as required.
Knowledge of primary care practices and procedures.
Strong skills in diagnostic evaluation to assess patient conditions accurately.
Familiarity with case management principles for effective coordination of care.
Experience in hospice care settings is a plus.
Competence in specimen collection processing techniques.
Excellent communication skills, both verbal and written, to interact effectively with residents, families, and healthcare teams.
Strong organizational skills to manage multiple tasks efficiently while maintaining attention to detail.
