Full Job Description

Job Description

The MDS Coordinator will be responsible for the overall coordination and completion of the RAI/MDS process to assure accurate completion of Resident Assessment in compliance with the federal and state requirements.

Responsibilities:

Maintains current MDS status of each resident, according to state and federal guidelines.
Schedules residents for appropriate Medicare PPS/Managed and/or Medicaid required assessment.
Communicates schedule to all other departments and to families.
Maintains system to monitor for changes in resident condition.
Responds to resident change in condition by coordinating resident reassessment and reevaluating plan of care.
Strategically sets assessment reference date (ARD), in conjunction with the interdisciplinary team, to capture needs, care and services delivered that most accurately reflect the status of the resident.
Assures compliance with federal, state, Medicare, Medicaid regulations regarding completion, accuracy, electronic submission, and timeliness.
Assist in the coordination and development of the plan of care as required.
Monitors MDS and care plan documentation on all residents, assuring that complete, orderly and chronological documentation is on file.
Communicates information to nursing administration, business office, and facility administration regarding assessments, care plans, updates and revisions.
Attends daily Case Management meetings.
Electronically transmits MDS data to the state per regulation.
Participates in QAPI meetings.

Qualifications:

Current Registered Nurse (RN) or Licensed Vocational Nurse (LVN) in good standing and currently licensed by the state of Texas.
Minimum Data Set and Care Plan experience.
Knowledge of Medicare and Medicaid programs.
Familiarity with Point Click Care a plus.
