We are currently seeking a full-time Physical Therapist (PT) to join a respected hospital-based rehabilitation team providing both inpatient and outpatient services. Position Summary
The Physical Therapist evaluates and treats patients across inpatient and outpatient settings to restore function, improve mobility, reduce pain, and prevent or minimize long-term physical limitations. Working as part of a multidisciplinary healthcare team, the PT helps patients of all ages recover from injuries, surgeries, illnesses, and chronic conditions while promoting optimal functional outcomes.
Key Responsibilities

Perform comprehensive physical therapy evaluations for patients with orthopedic, neurological, musculoskeletal, cardiopulmonary, and post-surgical conditions.
- Develop individualized treatment plans based on clinical findings, physician referrals, and patient goals.
- Provide evidence-based therapeutic interventions, including manual therapy, therapeutic exercise, gait training, balance training, neuromuscular reeducation, and therapeutic modalities.
- Educate patients and caregivers on home exercise programs, injury prevention, safety strategies, and lifestyle modifications.
- Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes.
- Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, case managers, and other healthcare professionals to coordinate patient care.
- Maintain accurate and timely documentation, including evaluations, daily treatment notes, progress reports, and discharge summaries within the electronic medical record (EMR) system.
- Ensure compliance with hospital policies, state licensure requirements, accreditation standards, and professional best practices.
- Participate in interdisciplinary team meetings and discharge planning activities.
Qualifications
Required

Doctor of Physical Therapy (DPT) or Master's Degree in Physical Therapy from an accredited program.
- Current and active state Physical Therapy license.
- Strong communication, organization, and clinical reasoning skills.
Preferred
-
Previous experience in a hospital, inpatient rehabilitation, acute care, or outpatient orthopedic setting.
- Experience treating a diverse patient population across multiple diagnoses.
- Knowledge of EMR documentation systems.
