Full Job Description

Administrative Coordinator

Myriad Homecare is seeking a motivated, professional, and organized Administrative Coordinator to join our office team. This position supports the day-to-day administrative operations of our home care agency while also assisting with patient referrals, community outreach, and efforts to identify and connect with potential new patients and referral sources.

The Administrative Coordinator will be responsible for coordinating patients and Nurse Aides, maintaining patient records, telephone coverage, appointment scheduling, registration, referrals, insurance verification, and assisting with community and referral outreach.

The ideal candidate will have at least three years of administrative experience, strong communication and organizational skills, and the ability to professionally represent Myriad Homecare when communicating with patients, families, case managers, community organizations, and potential referral partners.

Responsibilities

Guide potential patients and their families through the home care referral and intake process.
Respond to inquiries from prospective patients and families and conduct appropriate follow-up regarding available home care services.
Assist with identifying and developing potential new patient referral opportunities within the communities and counties served by Myriad Homecare.
Build and maintain professional relationships with potential referral sources, including CAP/DA case managers, community organizations, senior-serving organizations, healthcare providers, social workers, discharge planners, and other community partners.
Conduct periodic outreach and follow-up with referral sources to increase awareness of Myriad Homecare's services and determine whether individuals within their caseloads or communities may need in-home care services.
Maintain a list of prospective patients and referral opportunities and conduct timely follow-up as appropriate.
Assist management with community outreach activities, informational events, senior workshops, health fairs, and other opportunities designed to increase awareness of Myriad Homecare.
Communicate with CAP/DA case managers throughout the counties we serve to determine whether additional aide coverage or home care services may be needed for individuals on their caseloads.
Organize and maintain patient charts, prioritize documentation, and review charts for completeness and accuracy.
Communicate effectively and professionally with patients, families, caregivers, case managers, and other healthcare professionals.
Manage patient schedules and work cooperatively with Nurses and Nurse Aides to ensure appropriate and workable staffing schedules.
Verify insurance and program eligibility or coverage for planned services when applicable.
Assist with coordinating new patient referrals, intake documentation, and communication between patients, families, nurses, aides, and referral sources.
Resolve administrative issues by reviewing information, identifying potential solutions, and communicating effectively with the appropriate individuals.
Maintain accurate documentation of patient inquiries, referrals, outreach efforts, and follow-up activities.
Provide professional telephone coverage and courteous customer service to internal and external customers.
Perform additional administrative and patient coordination duties as assigned.

Required Qualifications

High school diploma or equivalent.
At least three years of administrative, healthcare, home care, customer service, or related experience preferred.
Proficiency with Microsoft Office Suite.
Excellent written and verbal communication skills.
Strong organizational and follow-up skills.
Ability to communicate professionally and compassionately with seniors, individuals with disabilities, patients, families, caregivers, and healthcare professionals.
Ability to establish and maintain professional relationships with community organizations and referral partners.
Professional appearance and commitment to providing courteous customer service to internal and external customers.
Ability to maintain patient confidentiality and handle sensitive information appropriately.
