August Healthcare is seeking an organized, responsive, and relationship-focused Centralized Admissions Coordinator to support the referral and admissions process across our skilled nursing and post-acute care centers.
This is an important role that helps connect prospective residents and families with the care they need while supporting our centers in achieving safe, timely, and appropriate admissions. The ideal candidate is a strong communicator who can manage multiple priorities, work collaboratively with clinical and operational leaders, and provide exceptional service to hospital partners, residents, families, and center teams.
As the Centralized Admissions Coordinator, you will serve as a central point of contact throughout the referral and admissions process. You will work closely with hospital case managers, discharge planners, referral sources, residents, families, center leadership, nursing teams, Business Office Managers, managed care personnel, and other members of the interdisciplinary team.
Key responsibilities include:
- Receiving, reviewing, tracking, and following referrals through final disposition
- Monitoring hospital referral portals, email accounts, telephone calls, and other referral channels
- Gathering missing clinical, demographic, insurance, and financial information
- Reviewing referral documentation for completeness and identifying potential admission barriers
- Coordinating clinical review with Directors of Nursing, Administrators, therapy teams, and other appropriate leaders
- Routing insurance and payer information to the appropriate Business Office, managed care, or authorization personnel
- Coordinating admission dates, arrival times, transportation, room availability, equipment, medications, and other admission details
- Providing timely admission notifications to receiving center departments
- Maintaining clear and professional communication with residents, families, referral sources, and center teams
- Supporting census and occupancy goals by helping reduce unnecessary delays in the admissions process
- Redirecting appropriate referrals to another August Healthcare center when needed
- Maintaining accurate documentation of referral activity, approvals, denials, follow-up efforts, and admission outcomes
- Participating in census, admissions, referral review, training, and performance improvement meetings
- Providing evening, weekend, holiday, or on-call support based on departmental needs and established schedules
We are looking for someone who is dependable, detail-oriented, and comfortable working in a fast-paced healthcare environment.
Minimum qualifications include:
- High school diploma or equivalent required
- Associate or bachelor’s degree in healthcare administration, business, social services, nursing, marketing, or a related field preferred
- Previous experience in skilled nursing, long-term care, healthcare admissions, hospital discharge planning, insurance verification, case management, medical office administration, or a related healthcare setting preferred
- Familiarity with Medicare, Medicaid, managed care, private pay, and other healthcare payer sources preferred
- Experience using referral management systems, hospital referral portals, electronic health records, or healthcare admissions platforms preferred
- Familiarity with skilled nursing facility admissions and post-acute care services strongly preferred
- Excellent verbal and written communication skills
- Strong organization and time-management abilities
- Ability to manage multiple referrals, deadlines, and competing priorities
- Strong attention to detail and documentation accuracy
- Sound judgment and the ability to recognize when escalation is necessary
- Professional and compassionate customer service skills
- Ability to work independently while remaining accountable to established processes
- Strong relationship-building and teamwork skills
- Ability to maintain confidentiality and handle sensitive information appropriately
- Proficiency with Microsoft Outlook, Word, Excel, Teams, and other standard business applications
- Ability to learn referral tracking systems, electronic health records, payer portals, and other healthcare technology platforms

This position involves frequent computer, telephone, email, video conference, and electronic system use. Additional hours, evenings, weekends, holidays, or on-call coverage may be necessary depending on referral and admissions activity.
