Program Overview:
We are not your typical private behavioral health agency. As a family-owned organization deeply rooted in our community, our mission is to provide compassionate, person-centered care to those who need it most, especially individuals covered by MassHealth, the Commonwealth’s Medicaid program. While we accept both public and private insurance, nearly 70% of our Clients are MassHealth recipients, and we treat each client with the dignity, respect, and the individualized attention they deserve.

Key Responsibilities:
As an Outpatient Services Clinician at SMG, you will play a vital role in delivering high-quality, person-centered behavioral health care to a diverse adult population. This position offers the opportunity to make a meaningful impact in the lives of Clients while working within a supportive, mission-driven team. Your responsibilities will combine clinical expertise with flexibility, creativity, and collaboration to ensure each member receives care that meets their unique needs and goals. The position blends responsibilities within both our Outpatient Clinics (Individual, Couple, Family Therapy; stand-alone group therapy) and our IOP/PHP program (milieu and group therapy, and case management).
1. Provide individual, couples or family therapy to adult Clients experiencing mental health, substance use, or co-occurring disorders, using evidence-based and person-centered approaches
2. Provide weekly group treatment in a stand-alone group to adult Clients experiencing mental health, substance use, or co-occurring disorders, using evidence-based and person-centered approaches
3. Provide IOP/PHP groups, including milieu treatment to adult Clients experiencing mental health, substance use, or co-occurring disorders, using evidence-based and person-centered approaches
4. Conduct comprehensive clinical assessments in compliance with SMG standards and payer requirements
5. Maintain accurate and timely documentation of all clinical encounters, treatment plans, and progress notes in accordance with SMG policies and MA regulations
6. Support Clients in achieving recovery goals by integrating community resources, coordinating care with medical and psychiatric providers, and addressing social determinants of health
7. Achieve Performance Metrics set by Manager
a. Deliver a full caseload each week
i. Number of Individual Therapy Sessions: up to an average of 25 sessions per week
ii. IOP/PHP: up to an average 12 groups per week (with case management for up to 12 Clients)
iii. The number of individual therapy sessions decreases based on the number of IOP/PHP group sessions and vice verse; for example, if 12 IOP/PHP group sessions are delivered in a week, then 10 individual therapy sessions are required for a full caseload
b. Number of Intakes to ensure caseload to achieve volume required in 4a: as scheduled
c. Attendance and active participation in internal meetings
d. Residential groups: as needed
e. On-time completion of paperwork for clinical encounters and billing: within 24 hours
8. Engage in clinical supervision and team meetings to support ongoing learning, case consultation, and professional development
9. As needed, deliver outpatient, IOP/PHP and/or residential group therapy sessions based on the developmental stage and evolving needs of each group, rather than following a fixed manual, ensuring person-centered care at every level.
10. Contribute to a positive, supportive team culture that values communication, compassion, shared learning, and a commitment to SMG’s mission
11. Uphold ethical, legal, and professional standards, including client confidentiality, mandated reporting, and compliance with licensing and accreditation requirements
Specific Tasks:
The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.
1. Conduct one-on-one (and as needed group) counseling sessions as needed to support individual client goals and treatment plans
2. Complete intake assessments and collaborate on treatment planning for new Clients entering the program
3. Maintain accurate attendance records and contribute to centralized data tracking systems (e.g., shared spreadsheets)
4. Coordinate collateral contacts with outside providers and supports (e.g., case managers, medical professionals, family members) to ensure continuity of care
5. Support Member discharges by preparing discharge summaries, coordinating referrals, and ensuring appropriate follow-up planning
6. Participate in daily team huddles and regular clinical supervision to review cases, adjust treatment plans, and maintain alignment with program goals
7. Engage in Member outreach efforts, including follow-up calls or messaging to support engagement and reduce no-show rates
8. Contribute to program operations by assisting with documentation workflows, quality improvement initiatives, and staff communication
9. Take scheduled breaks to support self-regulation and model healthy boundaries and wellness practices for Clients

- Masters Degree in Social Work, Mental Health Counseling, Art or Expressive Therapy
- Experience using Electronic Medical Records (EMR), experience, InSync preferred
- Previous experience working with adults with co-occurring mental health and substance use disorders
- Licensure preferred (LCSW, LICSW, LMHC)
- Group therapy experience preferred
- No on-call coverage required

- Full-time, 40 hours per week, Scheduling is flexible.
- This position is hybrid in the Greater Boston Area (Woburn and Weymouth).
