Full Job Description

Position Summary

The Day Habilitation Coordinator assists individuals served in becoming more self-sufficient, self-advocating, self-directed, and engaged in meaningful activities. The Coordinator is responsible for coordinating day-to-day site operations, staff supervision, person-centered service implementation, documentation, participant supports, incident response, and compliance with OPWDD and agency requirements.

Essential Functions

· Supervise, train, coach, and support Day Habilitation staff and program support staff.

· Assist with daily management of the site, including participant schedules, staffing coverage, activities, and safety.

· Orient new staff to the facility, participant needs, documentation requirements, and program expectations.

· Review, approve, and submit biweekly payroll and related staff records as assigned.

· Coordinate person-centered planning processes to identify participant interests, goals, community activities, support needs, and service outcomes.

· Ensure participants are supervised and supported in a safe, clean, respectful, and person-centered environment.

· Coordinate timely and efficient delivery of planned services, activities, transportation, and community-based programming.

· Attend programmatic staff meetings, departmental meetings, case reviews, and required trainings.

· Assist program leadership with implementation of the functional program, including philosophy, goals, objectives, and quality expectations.

· Coordinate and communicate with participants, families, residential programs, care managers, treatment providers, and referral sources.

· Coordinate on-site activities, community inclusion opportunities, recreation, skill development, vocational exposure, and habilitative supports.

· Support implementation of daily living skills, treatment plans, behavior supports, safety plans, and individualized service plans.

· Assist with psychiatric, medical, socialization, vocational/educational, and recreational support needs within the scope of the program.

· Ensure incidents are documented and reported to supervisors in a timely manner and in accordance with OPWDD, Justice Center, and agency procedures.

· Participate in incident investigation, non-reportable incident review, follow-up, and risk mitigation as assigned.

· Monitor staff attendance, staffing needs, participant enrollment, recruitment, and retention-related concerns.

· Promote positive behavior guidance, respectful interactions, and professional boundaries among staff.

· Complete support staff evaluations and provide feedback, coaching, and corrective guidance as assigned.

· Maintain participant records, program files, summaries, and supportive documentation accurately and timely.

Administrative Responsibilities

· Maintain required program records, participant files, attendance records, billing support, monthly summaries, and other documentation.

· Ensure documentation is completed accurately and in accordance with agency timelines.

· Act as liaison between Halo Network, participants, families, providers, and referral sources.

· Remain available for on-call or urgent client emergencies/crises as assigned and provide appropriate interventions within agency protocols.

· Assist with audits, surveys, quality assurance reviews, corrective action plans, and staff training initiatives.

Compliance Responsibilities

· Maintain confidentiality of employee, participant, family, program, billing, and organizational information.

· Comply with applicable OPWDD regulations, Medicaid documentation standards, person-centered planning expectations, and program-specific guidance.

· Comply with HIPAA and other confidentiality requirements when accessing, using, documenting, transmitting, or discussing protected or sensitive information.

· Comply with Justice Center and mandated reporting obligations, including timely notification of reportable incidents when applicable.

· Follow Halo Network policies and procedures, including standards for respectful communication, workplace conduct, and professional boundaries.

· Maintain accurate, complete, and timely records in paper and electronic systems.

· Report concerns, incidents, unsafe conditions, suspected abuse, neglect, exploitation, or other compliance issues through appropriate supervisory and agency channels.

· Participate in required trainings and maintain required certifications or clearances as applicable to the role.

· Use agency equipment, email, electronic systems, and records in accordance with policy and security requirements.

Required Education and Experience

· High school diploma or GED required.

· Four years of OPWDD, developmental disabilities, day habilitation, residential, or related human services experience preferred.

· Associate degree or bachelor degree in human services or related field preferred.

· Supervisory experience preferred.
