Position Purpose:
The Physician Assistant (PA) or Certified Nurse Practitioner (CNP) utilizes advanced knowledge and clinical expertise in providing primary and/or specialized health care services to individuals, families, groups and communities across the health-illness continuum. Diagnostic reasoning, advanced therapeutic interventions and education are key elements in the direct provision of care using a collaborative model. Duties include participation in coordination of care and inter-service and inter-team collaboration for specialty areas. The PA or CNP works closely with physicians and resident staff, patient care staff, patients and families to manage plan of care. The PA or CNP works as an autonomous practitioner or in collaboration with a physician, and is directly accountable for making clinical judgments to patient(s) using independent and interdependent decision-making skills. The PA or CNP writes medical orders (medications and tests) and procedures subject to internal and external rules and regulations of credentialing. The PA or CNP also functions as a consultant, educator, researcher, resource and role model for patient, families, community, nursing personnel and other health care providers. The PA or CNP evaluates the quality of care rendered; analyzes and promotes system changes to enhance quality of care and organizational effectiveness/efficiency; conducts/utilizes research to promote health and improve health care delivery. The PA or CNP assumes responsibility for own professional development. Appropriate privileges, licensure and certification are required to prescribe pharmacological and medical interventions.
Qualifications for Physician Assistant:
Master’s degree from an accredited College or University’s program for physician assistants and board certified required. Two to 5 years work experience as a Physician Assistant; previous experience in Pediatrics is preferred but not required. Excellent written, oral communication skills. Capacity to analyze and interpret data, and use ability to maintain confidentiality.
Certificates, Licenses, Registrations: State of Michigan Physician Assistant Licensure. Board Certified. DEA eligible.
Qualifications for Certified Nurse Practitioner:
1. Master’s degree in nursing from an accredited College or University.
2. Two to 5 years working as an RN; previous experience as an NP preferred but not required
Certificates, Licenses, Registrations: Registered nurse licensure to practice in the state of Michigan. State of Michigan Nurse Specialty Certification as a Nurse Practitioner.
Expectations and Responsibilities:
Under the limited direction of the attending physician or designee,
1. Responsible for night rounds on all hematology/oncology and BMT patients.
2. Orders, evaluates and monitors medical and laboratory diagnostic tests and
treatment and consultation plans. Responsible for following PM labs and intervene with blood products or electrolyte supplements. Check in with the Fellow on call to discuss issues.
3. Obtains and records patient medical history
4. Majority of the admissions include sickle cell patients with pain or fever. For patients with sickle cell, sometimes the fellow is called for treatment recommendations, other times they are not. The ED will call and sign out patients, usually the NP will complete some chart review prior to sign out to ensure an appropriate work-up has been completed prior to each patient coming to the floor. Admit our oncology patients, most common admissions are for fever or chemotherapy related side effects, such as nausea/vomiting/dehydration.
5. Communicate with fellow throughout the night.
6. Develops, prioritized, comprehensive problem list
7. Accompany patient(s) to diagnostic procedure.
8. Performs, documents, procedures (specifically delivery room attendance, reservoir taps, lumbar punctures, endotracheal intubation, peripheral inserted central line placement, umbilical line insertion, Intraosseous line placement, thoracentesis, chest tubes and exchange transfusions, abdominal paracentesis, moderate sedation)
9. Maintains active Neonatal Resuscitation Provider and/or PALS card as per institutional requirements.
10. Prepares, develops and initiates treatment plan in consultation with attending physician.
11. Documentations in Electronic Medical record.
12. Educates and counsels patient and family members on condition, desired results of treatment plans, expectations during illness and recovery, and other related areas.
13. Participates in teaching and patient management conferences.
14. Engage in quality improvement measures for safety and compliance
15. Coordinates and facilitates continuity of care that crosses institutional and organizational boundaries.
16. Acts as a patient/family advocate to ensure health care services are delivered in an ethical and culturally sensitive manner.
Working Conditions:
1. Expected to work (3) 12 hour night shifts and if coverage gaps 1 weekend day per block. Terms and conditions of the work schedule are flexible and may vary based on the needs of the department and are at the discretion of the attending physician or designee.
2. Occasionally lift and/or move up to 25 pounds with or without assistance.
3. Able to complete duties under stress, deadlines, and while attending to multiple duties simultaneously.
4. Prolonged computer related exposure, as well as sitting and standing at workstations for long periods of time.
5. Noise level in the work environment is usually moderate.
6. Bending, sitting, and standing frequently throughout the day.
7. Occasionally travel off-site for meetings, conferences, seminars, and training.
EXEMPT STATUS: This position is exempt from overtime pay provisions of the Federal Fair Labor Standards Act.
