Dedicated Clinical & Administrative Support
Each physician practices within a collaborative care team ("Pod") supported by a dedicated Regional Nurse Coordinator who serves as an extension of the physician's clinical team.
Regional Nurse Coordinators assist with:
- Daily scheduling and care coordination
- Documentation and note management
- Facility communication
- Prescription refills and medication-related requests
- Orders, forms, and administrative follow-up
- Communication with nursing staff, families, pharmacies, specialists, and other healthcare partners
This team-based model significantly reduces administrative burden, improves communication, and allows physicians to remain focused on clinical decision-making and exceptional patient care.

Our physicians typically see approximately 20 patient encounters per day, allowing adequate time for thoughtful clinical assessments, meaningful patient interactions, and evidence-based decision-making.

Position Responsibilities
The physician will provide comprehensive primary care and chronic disease management services to residents in Skilled Nursing, Assisted Living, and Independent Living communities.
Responsibilities include:
- Performing comprehensive patient evaluations
- Diagnosing and managing acute and chronic medical conditions
- Developing individualized treatment plans
- Collaborating with Advanced Practice Providers, nursing staff, specialists, facilities, families, and healthcare partners
- Providing leadership within an interdisciplinary care team
- Delivering evidence-based, patient-centered medical care
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Active Pennsylvania Medical License (or eligibility to obtain)
- Current DEA registration or ability to obtain
- Strong clinical judgment and communication skills
- Ability to practice independently while collaborating within a multidisciplinary team
- Commitment to providing compassionate, patient-centered care to older adults
Experience in Long-Term Care, Geriatrics, Family Medicine, Internal Medicine, Hospital Medicine, or Post-Acute Care is welcomed but not required.
