Position Summary
The PrEP Coordinator (LPN) provides community-based clinical services that expand access to HIV prevention, sexual health, and rapid treatment services for individuals disproportionately impacted by HIV. Working primarily in community settings and on mobile health units, this position conducts clinical assessments, performs laboratory specimen collection and CLIA-waived testing, supports rapid initiation of HIV pre-exposure prophylaxis (PrEP), post-exposure prophylaxis (PEP), and antiretroviral therapy (ART) under standing orders, administers medications within the scope of practice, and coordinates ongoing client care.
The PrEP Coordinator (LPN) serves as an essential member of the Community Health team by delivering culturally responsive, trauma-informed, and affirming services while collaborating with medical providers, community health staff, behavioral health professionals, and care coordination teams to improve health outcomes and reduce barriers to care.
Essential Duties & Responsibilities
- Conduct nursing assessments, HIV risk assessments, health screenings, and eligibility evaluations for PrEP, PEP, HIV treatment, and other prevention services.
- Perform CLIA-waived testing, rapid HIV/STI screening, rapid Hepatitis C screening, venipuncture, phlebotomy, specimen collection, processing, and laboratory documentation in accordance with established clinical protocols.
- Support rapid PrEP, PEP, and ART initiation under standing orders and provider supervision.
- Administer long-acting injectable medications and other prescribed treatments within LPN scope of practice.
- Review medication histories and provide medication adherence counseling and client education regarding sexual health, and HIV and STI prevention and treatment options.
- Coordinate care between clients, medical providers, pharmacies, insurance carriers, prescription assistance programs, and internal care teams.
- Assist clients with insurance navigation, medication access, prior authorizations, and linkage to ongoing medical care.
- Provide clinical services during community outreach events, mobile health clinics, and other off-site locations.
- Collaborate with community health workers, patient navigators, therapists, medical providers, and other interdisciplinary team members to ensure coordinated client care.
- Monitor clients for adverse reactions, follow-up needs, and retention in care.
- Complete accurate and timely documentation in the electronic health record and other required reporting systems.
- Maintain compliance with HIPAA, OSHA, CLIA, organizational policies, grant requirements, and applicable federal, state, and local regulations.
- Assist with quality improvement initiatives, inventory management, infection prevention practices, and development of Standard Operating Procedures (SOPs) related to community-based clinical services.
- Participate in interdisciplinary meetings, case conferences, staff development activities, and required trainings.
- Perform other duties as assigned.
Minimum Qualifications
Education
- Graduate of an accredited Licensed Practical Nursing (LPN) program.
Experience
- Minimum of two (2) years of nursing experience in HIV prevention, HIV treatment, infectious disease, sexual health, primary care, community health, preventive health, or related clinical setting.
- Experience providing services in community-based or mobile healthcare settings preferred.
Licenses/Certifications
- Current Licensed Practical Nurse (LPN) license in Maryland required.
- Current District of Columbia LPN license or ability to obtain within 90 days.
- Dual Maryland and District of Columbia licensure preferred.
- Current Basic Life Support (BLS)/CPR certification required.
- Valid driver's license with a driving record that meets the organization's insurance requirements.
Knowledge, Skills, and Abilities
- Knowledge of HIV prevention, PrEP, PEP, STI prevention, sexual health, and community-based clinical services.
- Knowledge of nursing principles, infection prevention practices, medication administration, and applicable standards of care.
- Ability to perform venipuncture, phlebotomy, specimen collection, CLIA-waived testing, and medication administration within scope of practice.
- Knowledge of electronic health records and clinical documentation standards.
- Ability to maintain confidentiality, comply with HIPAA and organizational privacy requirements, and maintain professionalism when working with clients.
- Strong communication, interpersonal, education, and relationship-building skills.
- Ability to work effectively with diverse populations while providing culturally responsive, affirming, and trauma-informed care.
- Ability to work independently in community settings and collaboratively within an interdisciplinary healthcare team.
- Strong organizational skills with the ability to manage multiple priorities and maintain accurate documentation in a fast-paced environment.
- Ability to exercise sound clinical judgment and recognize situations requiring provider consultation.
- Ability and willingness to work flexible hours, including evenings, weekends, and community outreach events.
- Ability to travel regularly throughout the service area and work on mobile medical units.
Preferred Qualifications
- Dual Maryland and District of Columbia LPN licensure.
- Experience administering long-acting injectable medications.
- Experience working in HIV prevention, Ryan White-funded programs, or federally funded public health initiatives.
- Bilingual English/Spanish and/or other languages strongly preferred.
Physical Demands and Work Environment
Work is performed in office, clinic, mobile health units, community settings, and field environments. This position requires frequent travel throughout Washington, DC , Prince George's County, Baltimore, and surrounding communities to provide clinical services during outreach events, mobile clinics, and partner locations.
The position requires the ability to sit, stand, walk, bend, perform venipuncture and specimen collection procedures, use computers and medical equipment, maintain clinical documentation, and communicate effectively with clients and interdisciplinary team members. Employees must be able to lift and transport clinical supplies and equipment weighing up to 30 pounds and work outdoors in varying weather conditions as needed.
