SUMMARY
The Vascular Advanced Practice Provider (APP) serves in a hybrid clinic and hospital role supporting the vascular surgery service line. The majority of the position is outpatient clinic-based, with regularly scheduled hospital responsibilities to maintain inpatient competency, continuity, and familiarity with hospital workflows. Depending on FTE and operational needs, the APP will generally spend one to two days per week in the hospital and the remaining time in clinic.
Hospital responsibilities include daytime rounding, inpatient and perioperative care, discharge coordination, and participation in surgical cases with the vascular surgeon assigned to the service that day. The APP also provides hospital coverage during planned or unplanned absences of other vascular APP team members. This position does not currently include overnight or weekend call. Work hours will generally fall between 7:00 a.m. and 5:00 p.m., based on clinic, rounding, and surgical schedules.
ESSENTIAL JOB FUNCTIONS
1. Provide comprehensive evaluation and management of vascular patients in the outpatient clinic, including history-taking, physical examination, diagnostic review, treatment planning, and follow-up care.
2. Provide scheduled daytime support for the inpatient vascular service, generally one to two days per week depending on FTE, staffing, and service-line needs.
3. Round on hospitalized vascular patients; assess clinical status; coordinate care with the attending vascular surgeon, nursing staff, hospitalists, and other members of the multidisciplinary team.
4. Support the vascular surgeon assigned to the daytime service, including participation in surgical or procedural cases as clinically appropriate and within the APP’s training, privileges, and scope of practice.
5. Provide inpatient coverage during vacations, leaves, and other absences of vascular APP team members to support continuity of service-line operations.
6. Manage pre-operative and post-operative care, including admission support, daily progress notes, medication management, discharge planning, discharge summaries, and follow-up coordination.
7. Develop and implement individualized treatment plans, including medical management, risk-factor modification, patient education, diagnostic imaging, and referral for vascular intervention when appropriate.
8. Order and interpret laboratory and diagnostic studies and prescribe medications and treatments within applicable licensure, credentialing, privileging, and organizational requirements.
9. Perform wound care and other office- or hospital-based procedures within the APP’s demonstrated competency, privileges, and scope of practice.
10. Provide clear and timely patient and family education regarding vascular disease, treatment options, procedures, recovery, and ongoing disease management.
11. Maintain accurate, complete, and timely documentation in the electronic medical record and comply with coding, quality, and regulatory requirements.
12. Collaborate closely with vascular surgeons, APP colleagues, clinic staff, hospital teams, and operational leaders to ensure seamless transitions and continuity of care.
13. Participate in service-line meetings, quality-improvement activities, education, and ongoing competency development related to vascular clinic and hospital care.
14. Adapt clinic and hospital assignments as reasonably needed to meet patient-care demands and vascular service-line coverage needs.

SCHEDULE AND PRACTICE SETTING
· Position may be structured as either 0.8 FTE or 1.0 FTE. Majority of scheduled time will be in the outpatient vascular clinic.
· Generally one hospital day per week for a 0.8 FTE position and one to two hospital days per week for a 1.0 FTE position, subject to staffing, patient volume, and service-line needs.
· Additional hospital coverage is expected during planned or unplanned absences of other vascular APP team members.
· Typical work hours fall between 7:00 a.m. and 5:00 p.m.; start and end times may vary based on rounding, clinic, and surgical schedules.
· No regular overnight or weekend call is currently required. Future schedule or coverage expectations would be communicated and addressed through the organization’s established process.
QUALIFICATIONS

EDUCATION and/or EXPERIENCE – NP/PA
· Graduate degree from an accredited nurse practitioner or physician assistant program required.
· Current Wisconsin and Michigan licensure, or eligibility to obtain Wisconsin and Michigan licensure, in one of the following professional pathways:
o Nurse Practitioner: Registered Nurse and Advanced Practice Nurse Prescriber licensure, as applicable to the position.
o Physician Assistant: Physician Assistant licensure.
· Current national certification appropriate to the applicable professional pathway required:
o Nurse Practitioner: National board certification in an appropriate adult or family practice population focus.
o Physician Assistant: Certification by the National Commission on Certification of Physician Assistants (NCCPA).
· Current prescriptive authority and all required federal and state registrations, or eligibility to obtain them, required.
· Experience in vascular surgery, cardiovascular surgery, inpatient surgery, or a comparable surgical specialty preferred.
· Surgical first-assist education or experience preferred. Candidates without prior first-assist training must be willing to complete any required education, training, or credentialing.
· Experience using Epic or a comparable electronic health record preferred.

· Clinical judgment necessary to identify normal and abnormal findings, prioritize patient needs, and escalate changes in condition appropriately.
· Ability to manage patients independently within scope while collaborating effectively with the supervising or collaborating physician and multidisciplinary team.
· Ability to transition effectively between outpatient clinic, inpatient rounding, perioperative care, and surgical environments.
· Strong organizational, time-management, documentation, and care-coordination skills.

· Commitment to high-quality, patient-centered care and continuous learning in vascular medicine and surgery.
· Knowledge of and compliance with scope-of-practice requirements, organizational policies, medical staff bylaws, confidentiality standards, HIPAA, and documentation requirements.
· Current BLS certification required; ACLS and other certifications may be required based on hospital privileging and organizational policy.
PHYSICAL/MENTAL DEMANDS and WORK ENVIRONMENT
The position requires standing and sitting for extended periods and may involve frequent bending or stooping and occasional squatting, reaching, crouching, pushing, or pulling. Lifting or carrying up to 25 pounds may be required. The role requires repetitive use of the hands, computer and medical-equipment use, visual attention, hearing for continuous communication and equipment monitoring, and the ability to make independent clinical decisions in a frequently stressful environment. The APP will work in outpatient clinic, inpatient hospital, procedural, and operating-room settings and may be exposed to the conditions ordinarily associated with those environments.
