As a Clinical Manager, you'll provide clinical leadership and support to our field team while helping ensure patients receive safe, compassionate, and high-quality home health care. You'll work closely with nurses, therapists, patients, families, and leadership to keep care coordinated and running smoothly.

What You'll Do

Lead, support, and mentor assigned clinical staff.
Oversee the delivery of high-quality, patient-centered home health services.
Coordinate patient care plans, recertifications, and discharge planning.
Lead multidisciplinary case conferences and promote strong communication across the care team.
Monitor clinical documentation, productivity, and overall staff performance.
Provide coaching, feedback, and support to help clinicians succeed.
Help ensure appropriate staffing and efficient scheduling.
Serve as a clinical resource for nurses, therapists, and other field staff.
Conduct supervisory and field visits as needed.
Support staff orientation, training, and ongoing professional development.
Partner with leadership on quality improvement and organizational initiatives.
Help ensure compliance with Medicare, state, federal, and agency requirements.
Step in to provide direct patient care when needed.

What We're Looking For

Current, active Registered Nurse (RN) license
2+ years of RN experience in home health or community-based care
Previous clinical management or supervisory experience preferred
Bachelor's degree in nursing or healthcare-related field preferred
Strong knowledge of home health regulations and Medicare Conditions of Participation
Excellent communication and leadership skills
Strong clinical judgment and decision-making abilities
Organized, dependable, and comfortable managing multiple priorities
Passion for supporting both patients and the clinicians providing their care

Working Requirements

Local travel and occasional patient/home visits required.
Reliable transportation and auto liability insurance required.
Ability to work a flexible schedule and participate in emergency call as assigned.
Ability to stand/walk for extended periods and lift up to 50 lbs. when making patient visits.
Ability to work effectively in a fast-paced environment and handle sensitive or stressful situations professionally.
Participation in required continuing education and training.
