Position Summary:

Responsible for the coordination and implementation of case management strategies pursuant to the discharge process. To provide efficient, quality discharge planning services to patients and their families from initial intake to discharge; to serve as a member of interdisciplinary team supporting the organization's treatment program and philosophy and assure the deliverance of quality treatment to patients and their families. Participates in performance improvement activities. Attends 80% of staff meetings. Coordinates care for patient through communication with Physicians, Nurse Practitioners, Clinical Services, Nursing, and Intake Department. Attends treatment team meetings as scheduled.

Position Requirements:

Required: Bachelors degree in a behavioral health field or business administration.
CPR and hospital-selected de-escalation technique certification
Maintains education and development appropriate for position
May substitute experience for education
Required: Minimum 1 year experience in acute hospital, home health, hospice, inpatient mental facility experience required (as applicable).
Required: Minimum 1 year experience in Discharge planning or Case Management.
Preferred: Prior experience in behavioral health setting as discharge planner/case management.

Position Responsibilities:

Clinical / Technical Skills (40% of performance review)

Requires knowledge of Federal and State regulatory agency standards related to health care organizations.
Analytical skills are required.
Must be able to demonstrate strong decision-making skills.
Builds and maintains strategic relationships.
Responsible for maintaining personnel files, including license verification and background checks.
Responsible for entering data into Human Resources Information System
Assist in processing new applications.
Assist in communicating with hospital leadership on position recruitment and onboarding
Must be able to communicate effectively with employees and applicants on all hospital Human Resources facets, including positions, benefits, and policies
Monitor and participate in hospital-wide orientation and Annual Education
Assist in monitoring the completion of onboarding and hospital education paperwork
Ensures the ordering of office supplies, to include required and requested SUN attire
Assist in conducting employee events, including Employee Socials, Town Halls/Open Forums, Open Enrollment, Orientation, and Annual Education.
Provides assistance to the Human Resources Director.
Perform other duties as assigned.
