Position Summary
The Registered Dietitian optimizes the nutrition status of PACE participants through comprehensive assessments, ongoing monitoring, and by providing clear communication of concerns and recommendations to the PACE Interdisciplinary Team.
How You’ll Make an Impact
- Perform nutrition assessments upon enrollment and re-assess annually; if/when significant changes in clinical status are noted; and as needed.
- Maintains food preferences and tray tickets for designated participants.
- Records periodic nutrition care data in the medical record.
- Reviews and adjusts therapeutic diets to reflect any changes in the daily menu and recommends the use of supplemental nutrition when necessary/appropriate.
- Assists in providing directions for meal service; educates and monitors staff to ensure that policies and procedures that will enforce safe food handling practices are being followed.
- Coordinates both inter- and intra-departmental professional activities to provide optimum nutritional care for the participants and serves as a consultant to clinical staff on matters of dietetics and nutrition.
- Attends and participates in Interdisciplinary Team (IDT) meetings as designated.
- Assists in the development and delivery of departmental and facility-wide educational programs.
- Ensures that all clinical issues and documentation meet and/or exceed Federal, State, and County regulations.
- Ensures compliance within the PACE pantries for the Department of Health surveys.
- Maintains rapport with participants, visitors, and family members and provides feedback on the overall PACE participant attitude regarding food service.
- Provides Medical Nutrition Therapy (MNT) to participants and/or caregivers as appropriate.
- Serves as dietitian liaison for PACE cases that are transitioned to other contracted facilities.
- Performs all other duties as assigned.
What You’ll Bring
This position may occasionally oversee dietary support staff, including pantry supervisors and activity assistants.
To perform this job successfully, an individual must be able to perform each primary function satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
- Bachelor of Science degree in Nutrition.
- One year of experience working with the geriatric population, preferred.
- Ability to teach and think analytically and relate to geriatric patients.
- Knowledge of the nutrition care process and federal, state, and local standards and regulations.
Certificates, Licenses, Registrations
- Active Registered Dietitian License in the State of Florida.
- Active Registration with the Commission on Dietetics.
