We are seeking a compassionate, organized, and motivated Bilingual (English/Spanish) Housing Case Manager to join our team. This is an opportunity to make a meaningful difference in our community.

A homeless housing nonprofit organization dedicated to helping individuals and families experiencing poverty and homelessness achieve stability through safe housing and supportive services. Case Manager Duties and Responsibilities

Conduct comprehensive client intakes and assessments to determine housing barriers, needs, and eligibility for services.
Develop individualized service plans with measurable goals and regularly review and update client progress.
Provide ongoing case management, advocacy, and support to help clients achieve housing stability and self-sufficiency.
Connect clients with appropriate community resources, benefits, healthcare providers, employment services, and other supportive programs.
Assist clients with completing applications for housing, employment, public benefits, rental assistance, vouchers, and other needed services.
Facilitate communication and maintain positive working relationships with landlords, property managers, and community partners.
Conduct home visits, housing inspections, and unit walkthroughs to monitor housing stability, safety, and program compliance.
Maintain accurate, timely, and detailed case documentation, including case notes, assessments, service plans, and required program records.
Monitor client progress, identify emerging needs, and modify service plans as appropriate.
Attend interdisciplinary meetings, case conferences, and community partner meetings to coordinate services and remain current on available resources.
Assist clients in obtaining essential identification and documentation, including birth certificates, driver's licenses, Social Security cards, SNAP/EBT cards, and other required records.
Maintain and update Coordinated Entry records and monitor the status of enrolled individuals experiencing homelessness.
Provide transportation or accompany clients to appointments, as program requirements permit.
Protect client confidentiality and ensure compliance with agency and grant policies.
Represent the organization professionally in all interactions with clients, landlords, community partners, and the public.
Demonstrate the ability to work independently while collaborating effectively as part of a team.
Communicate effectively through clear, professional verbal and written communication.
Perform other duties as assigned to support program operations and client success.

Pay: From $22.00 per hour
