Full Job Description

Job Summary

We are seeking a dynamic and experienced Post Acute Nurse Manager to lead our post-acute care program ensuring exceptional patient outcomes and seamless care transitions. In this vital role, you will oversee clinical operations within post-acute settings, coordinate multidisciplinary teams, and implement best practices to deliver high-quality, patient-centered care. Your leadership will foster a collaborative environment that emphasizes safety, compliance, and continuous improvement. The ideal candidate is passionate about patient care, possesses extensive clinical expertise across diverse healthcare settings, and demonstrates strong management skills to motivate and guide staff toward excellence.

Duties

Lead and coordinate daily operations within post-acute care program.
Oversee patient assessments, discharge planning, and clinical case management to ensure smooth transitions from acute care hospitals to post-acute environments.
Collaborate with healthcare professionals across disciplines—such as physicians, therapists, social workers, and case managers—to develop individualized care plans that meet patients’ medical and psychosocial needs.
Monitor compliance with healthcare regulations, policies (including HIPAA), and quality standards to maintain accreditation and ensure patient safety.
Supervise nursing staff and interdisciplinary team members; provide mentorship, training, and performance evaluations to promote professional growth.
Utilize Electronic Health Records (EHR) systems for documentation, medical records management, and communication of patient information securely.
Implement utilization management strategies to optimize resource use while maintaining high standards of patient care.
Conduct regular audits of clinical practices, infection control procedures—including infectious disease care—and safety protocols to uphold excellence in service delivery.
Engage in patient service initiatives by addressing concerns promptly, fostering positive client communication, and ensuring a compassionate approach to care.
Stay informed on health regulation policies related to post-acute services; adapt practices accordingly to ensure ongoing compliance.

Experience

Proven clinical experience in post-acute care settings such as skilled nursing facilities or outpatient clinics.
Extensive background in critical care environments including ICU, emergency medicine, Level I & II trauma centers, or similar high-acuity units.
Demonstrated expertise in managing complex cases involving medical-surgical conditions, geriatrics, behavioral health issues, or pediatrics as applicable.
Strong knowledge of hospital medicine practices including discharge planning, diagnostic evaluation, vital signs monitoring, and inpatient/outpatient coordination.
Familiarity with EMR/EHR systems for documentation and medical records management; understanding of HIPAA regulations is essential.
Experience with home health services or hospice care is advantageous for comprehensive post-acute management.
Ability to lead multidisciplinary teams effectively while managing patient assessments and clinical case management responsibilities.
Knowledge of health regulation policies related to managed care and inpatient/outpatient service delivery; commitment to continuous quality improvement.
