Overview

Join our compassionate healthcare team as a Certified Nursing Assistant (CNA) or Personal Care Assistant (PCA), where your dedication and skills will make a meaningful difference in the lives of our patients. In this vital role, you will provide high-quality patient care, assist with daily activities, and support individuals across various care settings, including assisted living, nursing homes, home healthcare, and hospice. We are committed to fostering a positive and energetic environment that values your expertise and encourages professional growth. All positions are paid, recognizing the importance of your contribution to patient well-being.

Responsibilities

Deliver personalized patient care by assisting with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support
Monitor and observe patients’ conditions through patient observation techniques, accurately documenting vital signs and medical information in electronic health record (EHR) systems
Assist with medical procedures such as catheter care, medication administration, and wound care while adhering to home healthcare regulations and safety protocols
Support patients with memory care needs, dementia care, Alzheimer’s care, and end-of-life care with compassion and patience
Safely transfer patients using appropriate patient transfer methods and equipment like Hoyer lifts or other assistive devices
Maintain patient confidentiality in compliance with HIPAA regulations while managing medical records and documentation
Engage in effective communication with clients, families, and healthcare teams to ensure comprehensive patient-centered care

Skills

Proven experience working with geriatric patients, individuals with developmental disabilities or disabilities requiring specialized caregiving
Strong knowledge of medical terminology, vital signs measurement, CPR certification, first aid skills, and basic life support procedures
Familiarity with EMR (Electronic Medical Records) systems and medical documentation practices
Ability to perform heavy lifting safely and manage physical demands associated with home health or long-term care environments
Excellent client communication skills combined with compassionate caregiving techniques for dementia or hospice patients
Experience assisting with ADLs such as cooking, laundry, cleaning, meal preparation, and general home health assistance
Knowledge of home & community care standards, hospice care protocols, and end-of-life support practices
