Care Manager - Home Care

Location: Holyoke, MA

Pay: $24.00–$26.00 per hour

We’re looking for a compassionate, motivated Case Manager to join our Home Care team in Holyoke, MA, serving clients in Holyoke, Chicopee, South Hadley, Granby, Ludlow, Belchertown, and Ware. What You’ll Do

As a Case Manager, you’ll be a trusted partner and advocate for consumers in our Home Care program. Your work will include:

Conducting thoughtful, person-centered assessments to understand each consumer’s strengths, needs, and goals (Visits are conducted in the client's homes)
Developing individualized care plans that support independence and quality of life
Coordinating services and community resources to ensure smooth, responsive care
Monitoring progress and adjusting plans to support the best possible outcomes
Advocating for consumer rights, needs, and preferences
Collaborating closely with a supportive, mission-driven team and community partners

Who You Are

You’re empathetic, organized, and driven by purpose. You bring:

A Bachelor’s degree in Social Work or a related field
Experience in case management, human services, or working with older adult populations
Knowledge of community resources and healthcare systems
Strong communication skills and the ability to build trust with consumers and families
Comfort with technology, including electronic health records
A valid driver’s license and reliable transportation
Ability to successfully complete required background checks (CORI, SORI, SAMs GSA, MassHealth Exclusion List, OIG)
