Full Job Description

Overview

Continuous Improvement Partners is conducting a confidential search on behalf of a client launching a new non-medical senior home care agency in Morgantown, serving Monongalia and Preston counties, with planned expansion into Marion and Harrison counties and into Garrett County, Maryland.

Our client is hiring the person who will run it.

This is a build role, not a caretaker role. You would be the senior operating leader from before the doors open- hiring the first caregiver cohort, standing up scheduling and after-hours coverage, carrying the agency through West Virginia Medicaid certification, opening two additional territories, and owning the P&L. By year three the plan calls for roughly 138 clients across three territories, about 70 caregivers, and $3 million in revenue.

What makes this different: this industry's failure is labor, not demand. Agencies turn away referrals because they cannot staff them, and caregiver turnover runs near 79% a year. Our client is built around fixing that- top-of-market caregiver wages, guaranteed hours in writing (no local competitor does this), paid orientation with an automatic 90-day raise, and a bench hired ahead of demand so the agency can say yes when a discharge planner calls at 4pm on a Friday.

The founder brings 15 years of healthcare strategy and corporate development experience at a Fortune-ranked healthcare company and remains actively involved in capital, compliance, and health system relationships. You would have a scheduler and a recruiter from month one, a community liaison from month three, and a per-diem RN for clinical assessments.

Compensation includes base salary, meaningful equity participation for the right operator, health benefits from day one with no waiting period, and a performance bonus tied to census, margin and caregiver retention.

WHAT YOU WOULD DO

Workforce- the core of the job

Own caregiver recruiting. From month 13 this business needs 7–8 gross hires per month, every month, and roughly half of any orientation class will not last. Building a machine that produces that reliably is the most important thing you would do.
Run paid orientation cohorts of 6–8, including competency testing and hands-on transfer technique check-offs.
Own retention: week-one and week-four calls, the 90-day raise calendar, referral bonuses, and the guaranteed-hours commitment.
Recruit channels most agencies ignore- senior centers, churches, CNA classes, community colleges.
Staff the weekend and evening shifts competitors refuse. That is the wedge.

Compliance and licensure

Carry the agency through WV Bureau of Senior Services certification for the Aged & Disabled Waiver, including the on-site office review and Medicaid enrollment.
Pass the six-month post-launch provider review and annual reviews thereafter.
Own WV CARES background checks and monthly registry rechecks on every employee.
Own incident reporting through WV IMS and the quality management program.
Support Personal Care Certificate of Need, Maryland RSA licensure, and case management certification.

Operations

Own scheduling and the 24/7 after-hours line, including on-call rotation and response standards.
Ensure every client has an RN assessment and written plan of care before service begins.
Manage EVV compliance and the agency platform.
Own service quality, complaints, and client retention.

Growth

Work the referral network with the community liaison: discharge planners, SNF social workers, elder law attorneys, care managers, primary care.
Build relationships with the area health systems and the local Medicare Advantage plan.
Own intake-to-start conversion. Speed of response decides whether an inquiry becomes a client.
Lead the Territory 2 and Territory 3 launches.

Financial

Own the P&L: revenue, gross margin per hour, billable utilization, overhead.
Manage payer mix across private pay, VA and Medicaid waiver.
Report monthly on census, hours, margin, headcount, turnover and pipeline.

WHAT OUR CLIENT IS LOOKING FOR

Required

5+ years running an operation in home care, home health, hospice, skilled nursing, assisted living, or a comparable business with a large hourly workforce
Demonstrated success recruiting and retaining hourly staff- be ready to talk specifics on headcount, turnover and what you did about it
P&L ownership, not just departmental budget responsibility
Comfort with state surveys, licensure and documentation standards
Scheduling judgment- you understand what covering a 6 am Sunday call-out actually takes
Valid driver's license and reliable transportation
Ability to pass a WV CARES background check

Strongly preferred

Direct experience with WV Medicaid ADW or Personal Care, BoSS certification, or the Acentra portal
Existing relationships with discharge planners or social workers at area hospitals and skilled nursing facilities
Experience opening a new location or territory, not only running an established one
Familiarity with WellSky, AxisCare, HHAeXchange or CareSmartz360, and EVV
Licensed Nursing Home Administrator, RN, LPN or comparable credential
VA Community Care contracting experience

Temperament

Bias toward hiring ahead of demand- the instinct to find someone rather than decline the referral
Comfortable building policies rather than administering them
Direct communicator who pushes back when they disagree
Genuinely likes caregivers. The people doing this work are the business.
