Job Summary

Join our dedicated healthcare team as a Home Health Aide and make a meaningful difference in the lives of patients requiring compassionate, personalized care in their homes. In this vital role, you will provide essential support to individuals across various stages of life, including seniors, those with disabilities, or patients recovering from illness or surgery. Your energy, empathy, and attention to detail will help promote independence, comfort, and well-being for our clients. This paid position offers a rewarding opportunity to develop your caregiving skills while working in a dynamic and supportive environment.

Duties

Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and toileting to ensure their comfort and dignity
Monitor patient health by observing vital signs like blood pressure, pulse, respiration, and temperature; report changes promptly to healthcare professionals
Support medication administration following prescribed schedules and medical documentation protocols while maintaining strict adherence to HIPAA regulations and patient confidentiality standards
Provide companionship and emotional support to reduce feelings of loneliness and promote mental well-being
Assist with mobility and transfers using patient transfer methods or equipment such as Hoyer lifts, ensuring safety at all times
Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment for clients
Document care activities accurately using Electronic Health Record (EHR) systems or Medical Records software; communicate effectively with team members about patient status and needs
Support clients with specialized care needs such as dementia care, Alzheimer's care, hospice support, or end-of-life assistance in accordance with home healthcare regulations

Qualifications

Proven experience working with individuals with disabilities or in assisted living environments; home health agency experience preferred
Knowledge of medical terminology, catheter care, medication administration, and vital signs monitoring
Certification in First Aid and Basic Life Support (BLS), including CPR training; additional certifications related to dementia or hospice care are advantageous
Ability to lift heavy objects safely and perform physical tasks required for patient transfer methods and mobility assistance
Strong communication skills for effective client interaction and documentation; familiarity with EMR/EHR systems is a plus
Compassionate caregiver committed to maintaining patient confidentiality under HIPAA guidelines
Experience working with geriatric patients or those requiring memory care or long-term care services is highly valued
Valid driver’s license and reliable transportation preferred for home visits
