Full Job Description

Occupational Therapy Supervisor (Per Diem)

Paradigm Rehabilitation is seeking an experienced and motivated Occupational Therapist to join our growing home-based therapy team as a Per Diem OT Supervisor, primarily supporting clinicians and patients in the Bronx.

This is an ideal opportunity for an OT who enjoys a combination of direct patient care, clinical mentorship, and supervision. The OT Supervisor will complete evaluations and reassessments, provide clinical oversight to a team of per diem Occupational Therapy Assistants (OTAs), review documentation, and serve as a clinical resource to support high-quality, compliant patient care.

As a per diem position, caseload and supervisory needs may vary based on patient referrals and agency census. We are looking for someone who is organized, responsive, and comfortable providing timely support to their assigned clinicians.

Responsibilities

Complete OT initial evaluations, reassessments, and supervisory visits within the assigned coverage area.
Provide clinical supervision, mentorship, and support to a team of per diem OTAs.
Review, approve, and co-sign OTA documentation in accordance with New York State, payer, and agency requirements.
Monitor patient progress and update treatment plans and recommendations as clinically appropriate.
Provide timely clinical guidance to OTAs regarding patient care, documentation, and changes in patient status.
Support continuity of care and collaborate with OTAs to address scheduling, caseload, and patient-related concerns.
Participate in case discussions, quality initiatives, and team meetings as needed.
Support the onboarding and ongoing clinical development of OTAs.
Collaborate with agency leadership and interdisciplinary team members to promote quality patient care and positive clinical outcomes.

Availability and Coverage

Per diem position primarily covering the Bronx.
Caseload and supervisory responsibilities will vary based on referrals and agency needs.
Flexibility to accommodate patient and OTA schedules is required.
Must be responsive to time-sensitive clinical and documentation needs for assigned clinicians.
Supervisory responsibilities may include oversight of up to 10 per diem OTAs, depending on caseload and agency census.

Qualifications

Graduate of an accredited Occupational Therapy program.
Active and unrestricted New York State Occupational Therapist license and registration.
Minimum of 2 years of clinical Occupational Therapy experience.
Previous supervisory and/or home care experience is strongly preferred.
Strong understanding of clinical documentation and Occupational Therapy practice requirements.
Excellent communication, organization, and leadership skills.
Ability to work independently while effectively supporting multiple clinicians and patient cases.
Comfortable using electronic medical records and clinical documentation systems.

Compensation

$80–$100 per session
Paradigm Rehabilitation provides administrative and clinical support from an experienced multidisciplinary team.
