Job Description: MUST HAVE CLINICAL EXPERIENCE!

The Nurse Supervisor is responsible for supervising and coordinating the delivery of skilled-nursing services in accordance with Colorado Department of Public Health and Environment regulations, Medicare Conditions of Participation, Medicaid requirements, agency policies, and accepted standards of nursing practice. The Nurse Supervisorensures the provision of high-quality patient care, clinical compliance, staff education, andquality improvement activities while promoting positive patient outcomes.

Responsibilities:

Supervise Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Home Health
Aides (HHAs), and other clinical staff.
Coordinate care for assigned clients and provide direct care as needed.
Perform on-site supervisory visits to assess the client, family, environment, and clinical caregivers.
Promote and manage expectations and satisfaction with internal and external customers.
Ensure patient care is delivered safely, effectively, and in accordance with physicianorders and individualized plans of care.
Provide nursing updates and obtain re-authorization for continued care.
Assist with providing ongoing supervision, orientation, training, education, and evaluation of clinical field staff.
Review and approve nursing documentation for accuracy, completeness, and regulatorycompliance.
Maintain a professional demeanor consistent with registered nurse standards of practice.
Base decisions and actions on ethical principles and foster a non-judgmental, non-discriminatory climate in which care is delivered in a manner sensitive to socio-cultural diversity.
Participate in on-call for after-hours client care.
Monitor patient outcomes and implement corrective actions when necessary.
Promote an environment of quality and safe client care through participation, development, and adherence to the QA plan and associated activities and metrics.

Qualifications:

One (1) year of in-field RN experience required
One (1) year of supervisory experience is preferred.
Current unrestricted Registered Nurse (RN) license in Colorado.
Home Health experience strongly preferred.
OASIS experience preferred.
Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
Ability to work with individuals to enlist cooperation of many people to perform/achieve a common goal.
Must be able to travel to patients’ homes in designated territory
Must have reliable transportation, valid driver’s license, and pass MVR check
Current CPR certification from AHA or ARC
Must be willing to supervise nurses providing in home skilled nursing care to adult

Environment:

Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions.
Possible exposure to blood, bodily fluids and infectious diseases.

Availability:

- Monday through Friday, 8am - 5pm in office

- Rotating weekends for on call and patient coverage
