Full Job Description

Job Overview
We are seeking a dedicated and organized Client Care Coordinator to join our medical team. In this role, you will be responsible for monitoring and supporting clients in a 6 - bed co - ed substance abuse facility, at DTX / RTC Levels of Care, as well as responsible for maintaining accurate records, all while fostering a welcoming environment.

Duties

Experience, knowledge or interest in chemical dependency and abuse.
Ability to communicate and work effectively with all staff and clients.
Mastery of evidenced based practices in medical care to residents in withdrawal/detoxification from substance use.
Ability to understand, empathize and work with an addicted population, including those with co-occurring disorders.
Knowledge of general medical procedures and techniques involved in the care of residents, disease processes and possible results, medical terminology, and facility routine.
Ability to keep records and prepare reports.
Strong ability to establish and maintain effective working relationships with the staff, clients, and authorized consents.
Proficiency in basic computer data entry skills, and experience with email.
Ability to accurately record resident information in the resident medical record and to notify appropriate personnel when deviations from the norm occur.
Ability to maintain confidentiality of residents' medical and personal information.
Assist with clerical tasks such as daily/monthly reports and resident record processing.
Oversee client self-administration of medications as prescribed by physician.
Ensures accurate documentation of medication orders.
Immediately and accurately receiving and transcribing orders from physicians (LVN or RN only).
Recognize individuals under the influence of alcohol and/or drugs and document such occurrences.
Respond to emergencies and initiate action as required.
Identifies resident symptoms requiring medical attention, or resident problems and behavior changes and communicates these to the medical director.
Accurately assess degree of detoxification or over sedation client may be experiencing during detoxification process. Be familiar with common detox assessments, CIWA and COWS.
Will maintain medication logs, counts, and client compliance of medications as well as monitor for medication adjustments or complications.
Audit medical room.

Requirements

Valid, Unrestricted Driver's License.
Active RADT - i, SUDRC, CNA, EMT, LVN, LPT Certification / License.
1 year of substance abuse experience (preferred).
CPR / First Aid Certification required.
KIPU EMR Experience (Preferred)
