Full Job Description

THE POSITION

DESCRIPTION OF WORK

This position helps older adults understand available supports and navigate important services. It involves detailed coordination, communication, and record keeping across multiple programs. As a Aging Case Aide 2, you will perform the following duties:

Application Support: Process Personal Care Home supplement applications and coordinate with agencies to resolve issues
Recertification Management: Track annual recertifications, send notices, and follow up with Personal Care Homes
Consumer Guidance: Provide clear explanations about services and assist with scheduling, referrals, and follow-up
Information Review: Check incoming consumer data for accuracy and refer complex concerns to caseworkers
Documentation Work: Record all case activities in required computer systems and maintain organized files
Information Sharing: Provide relevant program information and request or distribute materials as needed

Work Schedule and Additional Information:

Full-time employment
Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with 60-minute lunch.
Telework: You may have the opportunity to work from home (telework) part-time, hybrid work of up to two days per week after six months of employment. In order to telework, you must have a securely configured high-speed internet connection and work from an approved location inside Pennsylvania. If you are unable to telework, you will have the option to report to the headquarters office in Pittsburgh. The ability to telework is subject to change at any time. Additional details may be provided during the interview.
Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

One year as an Aging Case Aide 1; or
Two years of public contact work, including one year in a human services program; or
Any equivalent combination of experience and training.

Hiring preference for this vacancy may be given to candidates who live within Allegheny County. If no eligible candidates who live within Allegheny County apply for this position, candidates who reside in other counties may be considered.
You must be able to perform essential job functions.

Legal Requirements:

This position falls under the provisions of the Older Adult Protective Services Act.
- Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
You must pass a background investigation.
