Senior Care Manager – LPN

Premier Concierge Care Management
Location: Broward County, Florida
Job Type: Full-Time | W-2 | Non-Exempt
Compensation: $62,400–$70,720 annual equivalent, based on experience
Schedule: 40 hours per week

About the Role

Premier Concierge Care Management is seeking an experienced Licensed Practical Nurse (LPN) to join our South Florida team as a Senior Care Manager.

This is not a traditional bedside nursing position. It is an opportunity for an experienced LPN who enjoys care coordination, working independently, developing meaningful relationships with older adults and their families, and making sure important details do not fall through the cracks.

The Senior Care Manager will assume an established caseload and provide comprehensive care management, coordination, advocacy, monitoring, and family support. The position also requires flexibility to assist with new client assessments and time-sensitive care-management needs that may arise throughout the community.

We are looking for someone who is highly dependable, responsive, organized, proactive, and able to take ownership of a client's needs from identification through resolution.

What You'll Do

Manage an established caseload of older adult clients with varying medical, functional, cognitive, and psychosocial needs.
Conduct client visits and assessments in private homes, hospitals, rehabilitation facilities, skilled nursing facilities, assisted living communities, and other care settings.
Coordinate care with physicians, specialists, home health agencies, therapy providers, pharmacies, caregivers, facilities, and other members of the client's care team.
Monitor clients for changes in condition, functioning, services, safety, and overall care needs.
Follow through on physician recommendations, referrals, appointments, therapy orders, equipment needs, services, and other outstanding matters.
Maintain proactive communication with clients and authorized family representatives.
Support clients and families during hospitalizations, rehabilitation stays, transitions of care, and discharge planning.
Accompany clients to medical appointments when appropriate and coordinate necessary follow-up.
Develop, implement, monitor, and update individualized care plans.
Identify potential gaps in care and take appropriate action to address them.
Complete timely and thorough documentation of all care-management activities.
Conduct new-client assessments and assist with onboarding new clients.
Respond to time-sensitive client concerns and urgent care-management needs according to established coverage and escalation procedures.
Collaborate with company leadership and other members of the care-management team.
Develop positive professional relationships with healthcare providers, senior living communities, and other community resources.

Qualifications

Required:

Current and unrestricted Florida LPN license.
Valid driver's license and reliable transportation.
Ability and willingness to travel throughout Broward County and surrounding South Florida service areas.
Strong written and verbal communication skills.
Strong organizational, documentation, and computer skills.
Ability to work independently in community-based settings.
Ability to complete required background screening.

Preferred:

3+ years of relevant professional experience.
Geriatric care management or healthcare case-management experience.
Care coordination or transitions-of-care experience.
Home health, hospice, skilled nursing, rehabilitation, assisted living, memory care, managed care, or hospital experience.
Experience working with individuals with dementia and other complex chronic conditions.
Experience communicating directly with families, physicians, facilities, and interdisciplinary healthcare teams.

Schedule & Availability

This is a full-time, 40-hour-per-week position.

Most work is performed during regular business hours; however, the needs of older adults and their families are not always predictable. The Senior Care Manager must have reasonable flexibility to respond to time-sensitive client situations, including unexpected hospitalizations, discharge concerns, significant changes in care needs, facility concerns, caregiver disruptions, and other urgent care-management matters.

Occasional client needs may arise outside of the employee's regularly scheduled hours. All hours worked must be accurately reported and will be compensated in accordance with company policy and applicable wage-and-hour requirements.

This position does not replace emergency medical services. Medical emergencies requiring immediate intervention are directed to 911 or the appropriate healthcare provider.

Compensation

$62,400–$70,720 annual equivalent, based on experience

This is a full-time, W-2, non-exempt position, equivalent to $30.00–$34.00 per hour based on a 40-hour workweek. Overtime, when applicable, is compensated in accordance with applicable law.

Compensation within the range will be based on relevant experience, including geriatric care management, case management, care coordination, transitions of care, and experience working independently with medically complex older adults.
