SUMMARY OF JOB RESPONSIBILITIES

1. Develops a holistic, multi-disciplinary Plan of Care utilizing Clinical Pathway Practice Model.
2. Has 24-hour accountability for the coordination of care, implementation of physicians’ orders, and provision of care from admission to discharge.
3. Provides clinical supervision to the members of the Case Management Team (i.e., CHHA, RN, LPN, student).
4. Responsible for providing care and documenting it in a comprehensive, competent and thorough manner.
5. Available to work variable hours as necessary to meet the client care requirements.

QUALIFICATIONS

1. Graduated from an accredited school of nursing; licensed as a Registered Nurse in the state of practice; B.S. preferred.
2. Home care experience preferred.
3. Minimum of two (2) years nursing experience preferred.
