Full Job Description

About:

Harry Hynes Memorial Hospice was founded by community volunteers in 1983 as a nonprofit organization, we are honored to be a leading provider of Hospice and Palliative care in South Central and Southeast Kansas. Having served more than 40,000 patients and family members, we are experts in specialized care designed to provide support to you and your loved ones during an advanced illness. Our team of professionals, including a medical director, nurses, hospice aides, social workers, and volunteers, work in coordination with the patient’s physician to provide pain and symptom management, as well as social and spiritual support.

Summary

The RN Case Manager is responsible for managing the full continuum of patient care, including conducting admissions, completing comprehensive assessments, initiating and managing the plan of care, and coordinating ongoing services for assigned patients.

This role serves as the primary clinical decision-maker and point of accountability for each patient, ensuring timely admissions, high-quality care delivery, regulatory compliance, and effective communication with patients, families, physicians, and the interdisciplinary team.

Essential Function (Competencies)

1. Admissions & Intake Responsibility

Responds to assigned referrals and completes timely admissions.
Performs comprehensive physical and psychosocial assessments at admission.
Explains hospice services, including:
Medicare/Medicaid hospice benefit
Levels of care
Patient rights and expectations
Obtains all required consents and documentation.
Initiates the Plan of Care (POC) in collaboration with patient/family.
Communicates with attending physician to obtain orders and POC approval.
Accurately completes all EMR admission documentation same day.
Coordinates immediate needs (DME, medications, supplies).

2. Ongoing Case Management

Assumes full responsibility for assigned patient caseload.
Provides and/or coordinates
Routine visits
Symptom management
Patient/family education
Updates and manages Plan of Care continuously.
Ensures care aligns with patient goals and hospice eligibility.
Performs recertification assessments and documentation.
Maintains accurate, timely clinical documentation.

3. Care Coordination & Team Communication

Serves as the primary liaison between:
Physicians
IDT Teams
Facilities
Families
Participates in and contributes to:
IDT meetings
Care planning discussions
Coordinates services including:
HHA visits
Social work
Chaplain
Volunteers
Communicates changes in condition promptly.

4. Accountability for Quality & Compliance

Ensures documentation meets:
CMS Conditions of Participation
Agency standards
Maintains accuracy in data entry and clinical records.
Participates in:
Chart audits
QA activities
Identifies and escalates clinical or compliance risks.

5. Ownership of Workload & Professional Expectations

Manages daily schedule including:
Admissions
Visits
Follow-ups
Completes assigned work within scheduled shift
No routine deferral of responsibilities to other staff.
Demonstrates:

Maintains professional communication and collaboration

Education, Experience and Skills

Bachelor of Science preferred
Register Nurse License required.
Minimum 3 years nursing experience. Hospice experience is preferred.
Excellent written and verbal communication skills with strong attention to documentation.
Attention to detail.
Strong leadership skills.
Professional boundaries and friendly demeanor.
Organized and able to multi-task.
Advanced knowledge of Microsoft Office products.

Work Requirements

The job operates in a clinic environment and/or residential home-site. This role routinely uses computers, laptop, desk/mobile phones, photocopiers and fax machines.
Field-based role (homes, facilities, hospitals)
While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger and reach with hands and arms.
Frequent travel within service area.
Use of EMR systems, phone, and mobile technology.
