To be a successful Case Plan Coordinator you must be able to do the following always:

Assist the Permanency Services Teams with case plans, contract compliance, information gathering and tracking, database, and file maintenance. Ensure that Permanency Services Teams are compliant with case plan policies, time frames as established by Department of Children and Families (DCF), and quality content of case plans.
Ensure contract compliance with time frames for invite letters, facilitation, and completion/mailing of case plan as recommended by the case planning team. Collect signatures as required, complete case plan ongoing maintenance, ensure case plan updates are sent timely to participants, and ensure approved case plans are e-filed timely to court.
Participate in case plans as a neutral third party.
Timely staffing with Permanency Services Unit prior to case plan for updates. Provide due reports and follow-up around case plan and court hearing updates. Attend Permanency Services Teams Meeting to provide due reports and updates at least monthly. Participate in individual supervision at least monthly. Attend case plans as scheduled and Case Planning Team meetings as scheduled.
Accurately input and update case plans, court orders/hearings, client/family information in the electronic health record in a timely manner.
Complete additional tasks and duties as needed.
Follow Company Policy and Procedure and operate according to the Company’s Program Practice Guidelines.

To qualify for this job, you need to have:

High School Diploma or GED required.
Experience working with children and families preferred.
Strong organizational skills, time management skills, writing skills, and computer literacy.
21 years of age. Must pass a background check, fingerprinting, and physical and drug screening.
Maintain company-required training.
Physical working requirements - Ability to travel, driving personal vehicles including at night. Ability to do occasional lifting of up to 50 lbs. Able to walk stairs. Ability to work flexible schedule, including some evenings Employees are subject to outside environmental conditions occasionally, inside conditions frequently, with protection from weather conditions but not necessarily from temperature change.
