About:

Harry Hynes Memorial Hospice was founded by community volunteers in 1983 as a nonprofit organization, we are honored to be a leading provider of Hospice and Palliative care in South Central and Southeast Kansas. Having served more than 40,000 patients and family members, we are experts in specialized care designed to provide support to you and your loved ones during an advanced illness. Our team of professionals, including a medical director, nurses, hospice aides, social workers, and volunteers, work in coordination with the patient’s physician to provide pain and symptom management, as well as social and spiritual support.

Summary

The Nurse Triage & Float RN serves as a highly skilled, flexible clinical resource within the Hospice Command Center, responsible for providing triage support, responding to urgent patient needs, and delivering direct patient care across the full continuum of hospice services—from admission through end-of-life care.

This role functions as a deployable RN, supporting field operations, completing admissions, responding to call-offs, and managing urgent or complex patient needs. The Nurse Triage & Float RN collaborates closely with the Clinical Coordinator, RN Intake Coordinator, and Scheduler to ensure timely, high-quality, and uninterrupted patient care.

Essential Function (Competencies)

Clinical Care Delivery (Full Scope Hospice Care)

Provide comprehensive nursing care utilizing the nursing process:

o Assessment

o Planning

o Implementation

o Evaluation

Deliver care across the full hospice continuum:

o Admission

o Routine visits

o Symptom management

o End-of-life care

Develop and update individualized plans of care in collaboration with the interdisciplinary team

Triage & Symptom Management

Provide professional nursing triage services via phone and/or in-office coordination
Assess patient symptoms and provide clinical guidance and interventions
Determine need for escalation, visit, or additional services
Document all triage interactions per policy

Real-Time Deployment & Coverage

Remain available in-office as a deployable clinical resource
Respond to:

o Call-offs

o Coverage gaps

o Urgent patient needs

o Same-day admissions

Provide immediate support to maintain continuity of care

Admission Support

Complete hospice admissions as assigned
Conduct comprehensive admission assessments
Educate patients and families on hospice services
Initiate plan of care and documentation

Collaboration with Command Center

Work closely with:

o Clinical Coordinator (assignment and deployment)

o RN Intake Coordinator (patient readiness)

o Scheduler(s) (logistics and routing)

Provide real-time feedback on patient needs and operational challenges

Patient & Family Support

Provide compassionate, patient-centered care
Support patients and families through end-of-life process
Address emotional, psychosocial, and spiritual needs

Documentation & Compliance

Maintain accurate and timely documentation in EMR
Ensure compliance with hospice Conditions of Participation
Follow all clinical protocols and standards

Flexibility & Operational Support

Adapt to changing priorities and assignments throughout the day
Support team during high census or staffing shortages
Assist in maintaining efficient patient flow and care delivery

Education, Experience and Skills

Active Registered Nurse (RN) license – State of Kansas (or multi-state)
Minimum 2-3 years clinical nursing experience
Strong assessment and critical thinking skills
Ability to manage complex patient needs independently
Excellent written and verbal communication skills
Attention to detail.
Professional boundaries and friendly demeanor.
Organized and able to multi-task.
Hospice, oncology, critical care, or emergency nursing experience (preferred)
Experience with EMR systems (preferred)
Knowledge of hospice regulations and care standards (preferred)
BSN (preferred)

Work Requirements

The job operates in a in-office setting. This role routinely uses computers, laptop, desk/mobile phones, photocopiers and fax machines.
Frequent deployment to patient homes or care settings.
While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger and reach with hands and arms.
Requires real-time collaboration across multiple roles.
Fast-paced, high-demand clinical environment
Requires availability for urgent and time-sensitive situations
