JOB OBJECTIVE:
Assists the Director of Nursing (DON) in assuring that nursing service objectives, standards of nursing practice, and quality of care is maintained in the Home. Assures that the provision of care to each Resident meets their physical, social, emotional, and spiritual needs consistent with the comprehensive assessment and plan of care. Responsible for establishing and maintaining an infection prevention and control program designed to provide a safe, sanitary, and comfortable environment and to help prevent the development and transmissions of communicable diseases and infections. Also responsible for staff training, including planning overall orientation and ongoing education as well as in-service programs and safety/health teaching to Residents.
These services are consistent with the Home’s standards and philosophy and follow all regulations, codes, policies and procedures that govern the facility.
JOB STANDARDS:
· Must be a graduate of an accredited School of Nursing, licensed to practice in as a Registered Nurse.
· Must have a minimum of three years’ experience as a Registered Nurse.
· Must be able to assume the Director of Nursing’ responsibilities in her absence.
· Must possess good written and verbal communication skills. Must be able to read, write and understand English.
· Must be able to function independently performing related duties/tasks with a minimum of supervision or direction.
· Must have a thorough knowledge of State and Federal regulations and guidelines governing long term care facilities.
· Must possess maturity and leadership ability to interact effectively with staff, Residents, and families.
· Must possess management and supervisor skills to effectively assure that quality of care is provided and maintained.
· Possess the ability to perform the essential job functions with or without reasonable accommodations.
JOB RESPONSIBILITIES AND DUTIES:
Assistant Director of Nursing:
· Keep Director of Nursing informed of all nursing concerns and assist in problem solving.
· Review and evaluate all Residents' clinical records (in collaboration with the physicians) to assure the medical plan of care follows Federal and State regulations.
· Review nursing documentation, i.e. nursing summaries, follow-up charting, acute changes in condition, daily unit census, etc. to assure compliance with regulations and that charting presents an accurate, comprehensive and objective clinical picture.
· Coordinate physician visits according to schedule as well as daily physician calls after consultation with unit nurses.
· Assist Resident with issues related to psychosocial needs, i.e., aging and health changes/illness, loss and grief. Frequently visit Residents to assure optimal care, assessing and documenting changes in condition and informing of physician and family.
· Review all consultations, assure that follow-up is complete.
· Assure that lab, x-ray, and outside consultants are reported to the physician as warranted.
· Oversee the completion of the comprehensive Resident assessment and assure carry over/update to the plan of care in a timely manner to assure its accuracy and appropriateness.
· Assure completion of new admission and readmission order. Audit all closed clinical records, assuring complete and accurate documentation.
- Assure follow up on unit of rehabilitation/restorative measures. Communicate with therapy consultants regularly.
· Communicate with pharmacy frequently to assure staff compliance with Pharmacy Policy and Procedure and State and Federal regulations.
· Monitor medical supplies and equipment to assure safety and adequacy of care is given.
Strive for optimal use of supplies, materials; keeping in mind cost and budget limitations.
· Maintain confidentiality of all Resident, departmental and organizational information.
· Work effectively and cooperatively in role as leader/supervisor within department and with all departments or other disciplines.
· Attend in-services and department meetings to continually refine clinical skills.
· Respond calmly, quickly to Resident and employee emergencies and report to the Director of Nursing.
· Review Nurse Assistant documentation to ensure it correctly reflects care given and complies with regulation.
· Be flexible with schedule to meet needs of the Nursing Department.
· Observe dress and conduct codes, including wearing identification badge.
· Complete duties in prescribed time.
· Perform other duties as assigned by supervisor, which may include unit nurse duties.
The Staff Development Coordinator:
· Collaborate with department supervisors in developing in-service programs.
· Plan, coordinate and conduct regular and special in-service training programs for staff collaborating with the Director of Nursing to familiarize them with new procedures, policies or changes in resident care techniques and equipment; prepare lesson plans, evaluation, follow-up and record attendance at all in-services.
· Plan, implement and evaluate educational programs for all departments, collaborating with the Nursing Home Administrator and Director of Nursing regarding topics and methods of instruction.
· Review annually, or more frequently as needed, the job- related tasks to ensure that tasks involving exposure to blood/body fluids are properly identified.
Infection Control Nurse:
· Oversee infection prevention and control practices between all departments (includes universal precautions and in-service programs as required
· Compile clinical data and actively assist in research studies related to infection prevention and control. Prepare and present monthly summaries of all Resident infections, and quarterly report data to the assessment and assurance committees
· Collaborate with the Nursing Home Administrator, Director of Nursing and Director of Human Resources to develop, implement, and maintain an effective orientation program for new employees focused on infection prevention and control policies and procedures;
· Monitor infection prevention and control practices and procedures to ensure that all personnel are implementing appropriate procedures
· Follow all safety, security, infection/exposure control and hazardous waste policies and procedures (including Category I universal precautions).
WORK SETTING/ENVIRONMENT:
Work is performed primarily indoors but at times may attend meetings/seminars outside the facility or may participate in outdoor activities of the Home. Work environment is well lighted, clean and suitable to tasks performed by the employee within temperature control standards that govern a skilled care facility.
EQUIPMENT OPERATED:
Medication cart, stethoscope, blood pressure apparatus, blood sugar device, nebulizer apparatus, thermometer, needles, syringes, telephone, pacemaker equipment, oxygen, medications, hoyer lift, computer, fax machine.
PHYSICAL AND COGNITIVE REQUIREMENTS TO PERFORM THE ESSENTIAL JOB FUNCTIONS:
· Able to walk and stand constantly; frequently climb stairs; squat; kneel; twist; sit; bend and reach from floor to overhead.
· Able to frequently push and pull objects weighing 50 lbs. Must be able to push occupied wheelchairs.
· Able to frequently lift and move objects weighing 50 lbs., and occasionally lift and move objects weighing more than 100 lbs. This includes the occasional positioning and moving or Residents from all positions. In emergency situations, must be able to bear entire weight and move Resident to safety.
· Must have sufficient vision and color discrimination to observe Residents' conditions, read care plans, charts and other documents, read call light panels, instructions, color coded medication boxes and labels, thermometers, etc.
