Job Summary
We are seeking compassionate and dedicated Home Health Aides to provide exceptional in-home care to clients across various settings. The ideal candidate will assist individuals with daily living activities, ensure their safety, and promote independence. This role offers an opportunity to make a meaningful difference in the lives of patients while working in a supportive and professional environment.

Duties

Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
Monitor and observe patient health status, reporting any changes or concerns to healthcare professionals
Administer medications and assist with medication management following prescribed instructions
Perform vital signs assessments including blood pressure, pulse, temperature, and respiration rates
Support patients with memory care needs, including Alzheimer's and dementia care strategies
Operate and document care using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems in compliance with HIPAA regulations
Assist with light housekeeping tasks such as laundry, cleaning, and meal preparation to ensure a safe and comfortable environment
Utilize specialized equipment such as Hoyer lifts for patient transfers and catheter care as needed
Provide hospice or palliative care support in accordance with care plans
Maintain accurate medical documentation and records of patient interactions and treatments

Qualifications

Prior experience in home health, assisted living, nursing homes, or long-term care preferred
Knowledge of medical terminology, medication administration, and medical documentation practices
Certifications such as CPR are highly desirable
Experience working with individuals with developmental disabilities or disabilities is a plus
Ability to work independently while following care plans and protocols
Strong communication skills and compassionate bedside manner
Ability to work with diverse populations including seniors, individuals with dementia or Alzheimer's, and those requiring hospice or memory care services
Valid driver’s license preferred for travel between client locations
Must pass background checks and adhere to HIPAA privacy standards
Flexibility to work various shifts including live-in employment options is advantageous
