Full Job Description

Join our dedicated healthcare team as a Home Health Aide (HHA) - Floater! You will ensure high-quality care across multiple client locations (areas include: Hudson,Holliston, Westborugh,, Wayland, Framingham) by providing oversight, support, and coordination for caregivers and clients. This role conducts routine check-ins, monitors service delivery, documents observations, and communicates concerns to leadership.

The position also provides interim HHA services for new or transitioning clients until a permanent aide is assigned, ensuring continuity of care. Travel Reimbursement/Mileage reimbursement or gas card is available for approved work travel.

Ideal for someone who thrives in a flexible, fast-paced environment and is passionate about both caregiving and coordination.

Responsibilities

Client and Caregiver Support

Conduct regular in-person and phone check-ins with clients and assigned caregivers to ensure satisfaction and quality of care
Assess client needs, changes in condition, or environmental concerns and report findings promptly
Provide hands-on support, guidance, and problem-solving assistance to caregivers
Deliver interim direct care services when coverage gaps arise
Assist clients with Activities of Daily Living (ADLs) as needed

Quality Assurance and Monitoring

Verify that caregivers are following individualized care plans and agency policies
Perform spot checks on service delivery to ensure compliance with program standards
Identify gaps in care and collaborate with leadership to implement corrective actions
Maintain accurate documentation of visits, service observations, and follow-up actions

Communication and Coordination

Communicate urgent issues, safety concerns, or incidents immediately through proper channels
Support scheduling adjustments and coverage needs when necessary
Facilitate clear, professional communication to maintain continuity of care

Documentation and Reporting

Maintain detailed logs of client interactions, caregiver support, and quality monitoring activities
Prepare routine reports summarizing trends, issues, and service outcomes
Ensure all documentation meets regulatory, program, and confidentiality standards

Qualifications

Valid driver’s license and reliable transportation
Must be flexible to travel to the following areas: Hudson,Holliston, Westborugh,, Wayland, Framingham
High school diploma or equivalent required; additional coursework in health, human services, or related field preferred.
Prior experience as a Home Health Aide, Personal Care Aide, or in a coordination or supervisory role within home care and experience assisting with Activities of Daily Living (ADL's) preferred
Strong communication, interpersonal, and conflict-resolution skills
Ability to work independently, travel between client locations, and manage changing priorities.
Basic computer skills for documentation and scheduling systems.
Experience with Electronic Visit Verification (EVV) systems is a plus.
