Full Job Description

The Case Manager provides direct support and comprehensive case management services to individuals and/or families experiencing homelessness and disabling conditions to include behavioral health and substance use issues. Households are located throughout Worcester County.

The Case Manager works collaboratively with clients to reduce barriers to housing, increase stability, and support self-sufficiency through trauma-informed, client-centered practices. Case Manager connects clients to behavioral health services and assists with managing those conditions in housing. This position requires strong advocacy skills, cultural humility, and knowledge of local housing and social service systems. Case Manager has knowledge of Section 12, Section 35, Roger's Orders and representative payee services. Experience working with clients with acute psychiatric and substance use disorders.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Provide individualized case management to clients experiencing homelessness, including intake, assessment, goal planning, service coordination, and follow-up
Develop person-centered service plans in collaboration with clients, focusing on housing stability, income, health, and wellbeing
Build trusting relationships with homeless people that may be actively engaging in substance use and/or have under treated psychiatric conditions
Assist clients with housing search and placement, including applications, landlord communication, and coordination with housing providers
Support clients in accessing and maintaining community resources and benefits (e.g., MassHealth, SNAP, SSI/SSDI, employment services, behavioral health supports)
Responds to client crisis with the appropriate recommended level of care in the mental health and substance use continuum.
Works with psychiatric providers to monitor Roger's Orders and representative payee services.
Support clients to engage in a level of employment or volunteerism that is consistent with their capabilities
Maintain accurate and timely documentation and case notes in HMIS and agency databases in accordance with state and federal guidelines
Participate in Coordinated Entry and collaborate with Continuums of Care (CoC), shelters, outreach teams, and partner agencies
Provide advocacy on behalf of clients with landlords, service providers, and state agencies
Monitor client progress, reassess needs, and adjust plans as necessary
Participate in team meetings, case conferences, training, and supervision
Uphold client confidentiality and ethical standards at all times
Perform other related duties as assigned

Qualifications:

Required:

At least 3 years of experience working with individuals experiencing homelessness or housing instability and are experiencing behavioral health and/or substance use disorders.
Knowledge of Massachusetts social service systems, housing programs, and community resources
Knowledge of trauma-informed care, harm reduction, and motivational interviewing
Strong interpersonal, organizational, and documentation skills
Ability to work independently and as part of a multidisciplinary team
Valid driver's license and reliable transportation

Preferred:

Experience with HMIS and Electronic Health Records
Bilingual (English/Spanish or other languages commonly spoken in the community)
Lived experience or strong understanding of homelessness and housing insecurity
Knowledge of Section 12, Section 35, Roger's Orders and representative payee services.

Pay: $24.00 - $27.00 per hour
