MDS Nurse – RN/LPN

Company Description:

Blaire House of Milford is a 73-bed skilled nursing center. We have been family owned and operated, servicing the health care needs of seniors for nearly 50 years. Blaire House of Milford is a Five Star Quality Rated Nursing Facility. We specialize in Short Term Care, Long Term Care, Rehabilitation, Sub-acute Care, and Alzheimer's Specialty Care. Our facility also hosts an Adult Day Health Center, Senior Transportation and Home Care Services.

Blaire House of Milford is seeking an energetic MDS Nurse to uphold our high standards for senior care!

Job Summary:

1. Communicate, facilitate, instruct, direct, monitor, evaluate, and lead the assessment process.

2. Coordinate gathering and sharing of clinical information for the MDS and RAI process.

3. Monitor documentation to support payor claims.

Qualifications:

1. EDUCATION: R.N. or L.P.N. graduated from an accredited, approved school of nursing. Current Massachusetts registration with the State Board of Nurse Examiners.

TRAINING AND EXPERIENCE: Must have previous experience working on a nursing unit in a long-term care facility. Must have the understanding of all regulatory requirements in the area of documentation including but not limited to all state and federal regulations. Must understand OBRA, Medicare, Managed Care and case mix requirements. Knowledge of MDS, RAI and Care Planning Process. Must have excellent communication skills and possess team leadership. Must be able to meet deadlines and have computer literacy.

Salary Range: $90,000 - $95,000

Sign-On/Retention Bonus: $4,000
